生长激素治疗肝硬化低蛋白血症34例疗效观察

摘　　要：目的观察重组人生长激素(r-hGH)对肝硬化患者蛋白质代谢的影响。方法对68例肝硬化患者在保证热量的综合治疗前提下,随机分为r-hGH组及对照组。观察两组患者治疗前后的临床症状,检测治疗前、治疗后、停药2周后血清白蛋白、肝肾功能等指标的变化,检测治疗前内源性生长激素水平。结果肝硬化患者血清内源性生长激素水平升高[(15.8±3.2)ng/L],治疗10d后,患者食欲不振、腹胀、乏力、恶心等症状改善率高于对照组(P<0.05,P<0.01,P<0.01,P<0.05);治疗后及停药2周后血清白蛋白明显升高[分别为(31.9±4.2)g/L、(32.4±2.8)g/L],肝肾功能等指标无明显变化。结论在综合治疗基础上,使用r-hGH能迅速改善肝硬化患者的症状,改善低蛋白血症和营养状况,无明显不良反应。Objective To study the effect of treatment with recombinlng human growth horm＇one（r-hGH） in liver cirrhosis patients with hypoalbuminemia. Methods With integrating treatment（no blood products,albumin, albumin promoting drugs）,68 liver cirrhosis patients were divided into r-hGH group and control group. The change of clinic symptoms, serum albumin, liver functions, kidney functions were observed before treatment, in treatment stopping and in 2 weeks after treatment. The levels of endogenesis r-hGH were examined before treatment. Results The serum levels of endogenesis r-hGH in parients with liver cirrhosis were higher than normal（normal level〈 10ng/ L）. The rate of patients＇ inappetency, abdominal distention, tired, nausea decreased more in r-hGH group than control group（ P 〈 0.05, P 〈 0.01, P 〈 0.01, P 〈 0.05, respectively）. When treatment sropplng and 2 weeks after treatmenr,serum albumin incrcased significanrly（P 〈 0.01 ） in r-hGH group. Liver functions and kidney functions didn＇t change. Conclusion With integrating treatment, using r-hGH can decrease the symptoms of patients with liver cirrhosis rapidly,improve serum albumin and nutrition, r-hGH has no severe side effect.
