血清胃蛋白酶原Ⅰ和幽门螺杆菌抗体IgG与老年人胃黏膜萎缩相关性研究

摘　　要：目的探讨血清胃蛋白酶原Ⅰ和幽门螺杆菌抗体IgG与老年人胃黏膜萎缩关系。方法选取老年患者123例,慢性非萎缩性胃炎61例,慢性轻度萎缩性胃炎33例,慢性中-重度萎缩性胃炎29例,用ELISA法定量检测血清PGI的浓度和幽门螺杆菌IgG。结果在HP-IgG阳性和HP-IgG阴性患者中,慢性中-重度萎缩性胃炎组中血清平均PGI浓度明显低于慢性非萎缩性胃炎组和慢性轻度萎缩性胃炎组,差异有显著性。慢性非萎缩性胃炎组中血清平均PGI浓度与慢性轻度萎缩性胃炎组差异有显著性。同一组中的血清平均PGI浓度在HP-IgG阳性和HP-IgG阴性中差异无显著性。慢性非萎缩性胃炎、慢性轻度萎缩性胃炎和慢性中-重度萎缩性胃炎患者HP-IgG阳性率分别是65.57%、45.45%、51.45%,各组间HP-IgG阳性率差异无显著性。结论无论有无HP感染,血清PGI水平与胃黏膜萎缩程度呈阳性线性关系。通过检测PGI浓度来评估老年人胃黏膜萎缩程度和对胃癌早期诊断。Objective To investigate the relationship between serum pepsinogen Ⅰ levels as well as anti - HP IgG and elder patients with atrophic gastric mucosa. Methods 123 elder patients were selected in the study incIuding 61 cases of chronic nonatrophic gastric, 33 cases of chronic mild atrophic gastric and 29 cases of chronic moderate - severe atrophic gastric, serum pepsinogen Ⅰ levels and anti - HP IgG were detected by quantitative ELISA kits. Results With positive and negative anti - HP IgG , serum pepsinogen I leveIs of in chronic moderate - severe atrophic gastric group was significantly lower than of other groups. There were no obvious difference of serum pepsinogen I leveIs between positive anti - HP IgG and negative anti - HP IgG in the same groups. The positive rate of anti - HP IgG in chronic nonatrophic gastric, mild and moderate - severe atrophic gastrics where shown 65.57 %, 45.45 % and 51.45 %. There were no obvious difference of positive rate among three groups. Conclusion Whether positive or negative anti - HP IgG, serum pepsinogen Ⅰ is closely related with the extent of atrophic gastric mucosa . The serum pepsinogen Ⅰ measurement may be estimate the extent of atrophic gastric mucosa in elder patients and to raise diagnosis earIy gastric cancer.
