超声引导高温蒸馏水加干扰素瘤内注射治疗肝癌的研究

摘　　要：目的探讨超声引导瘤内注射高温蒸馏水及干扰素治疗肝癌的安全性、疗效及免疫学变化。方法对36例肝癌患者57个肿瘤,在超声引导下经皮肿瘤穿刺注射高温蒸馏水(90℃)加干扰素治疗,观察治疗效果及免疫学变化,并与CT、彩色多普勒超声及实验室检查情况相对照。结果超声见肿瘤缩小32个(56.1%);彩色多普勒超声肿瘤内血流消失46个(80.7%);肿瘤内血流明显减少8个(14%);57个肿瘤均行超声引导下活检,病理结果为大片状坏死组织,肿瘤完全坏死49个(86.0%),肿瘤不完全坏死8个(14%)。36例治疗后1个月行CT检查,肿瘤体积缩小31个(54.4%),增强CT显示肿瘤区内无明显强化,呈低密度;血清免疫球蛋白IgA、IgG、IgM和补体C3、C4及T淋巴细胞转化试验均有明显升高。结论应用超声引导下高温蒸馏水加干扰素瘤内注射治疗肝癌效果显著,安全性高,能提高免疫功能,是一种值得深入探讨的治疗方法。Objective To explore the safety, efficacy and immunological changes of the ultrasound-guided percutaneous hyperthermal distilled water （ HDW ） and interferon （ IFN ） injection for hepatocarcinoma patients. Methods In 36 hepatocarcinoma patients, 57 tumor masses, HDW and IFN were injected together. The treatment efficacy and immunological changes were observed with CT scan, colour Doppler ultrasonography and related experimental examination. Results The volumes of 32 tumor masses were decreased on ultrasonography, vascular supply disappeared in 46 masses on colour Doppler ultrasonography and vascular supply were diminished greatly in 8 masses. Ultrasound-guided biopsy were performed in 57 masses appearing massive area necrosis： completely necrosis in 49 masses and imcompleted necrosis in 8 masses. All patients received CT scan 1 month after treatment ： tumor volumes were decreased greatly in 31 masses and no enhancement with low density were revealed in the masses on enhanced CT. Immunoglobin IgA, IgG and IgM, complement C3 and C4, and T cell translation rate were increased. Conclusions The HDW and IFN injection provides better efficial and safe treatment. This is an ideal treatment method which can strengthen immune function beneficial for metastatic and weak hepatocarcinoma patients who lost surgical indication.
