眼眶淋巴瘤的MR影像特点

摘　　要：目的探讨眼眶淋巴瘤的MR影像特点,以提高其术前诊断准确率。方法回顾性分析8例经手术病理证实的眼眶淋巴瘤的MR影像学表现。结果7例为单侧,1例为双侧;4例位于肌锥外呈铸型;4例累及肌锥内外呈弥漫生长,其中3例累及视神经;8例均起源于泪腺和/或眼睑组织,并均有眼外肌受侵;1例有骨质破坏;平扫信号均匀,6例T1W I呈等信号,2例呈稍高信号,7例T2W I呈稍高信号,1例T2W I呈等信号;8例增强后均呈中等度均匀强化。结论眼眶淋巴瘤的MR影像学特征有助于提高其术前诊断准确率。Objective To study the MR features of orbital lymphoma in order to improve preoperative diagnostic accuracy. Methods MR manifestations in 8 cases of orbital lymphoma confirmed by pathology were retrospectively analyzed. Results Unilateral masses were found in 7 cases and bilateral masses in I case. There were 4 cases of extraconal masses and 4 cases of intra - and extra - tonal masses in which 3 showed the mass involving the optic nerve. All the masses arised from lacrimal gland and/or eyelid tissues and the extraocular muscles were involved in 8 cases. In comparison with extraocular muscles,the signal of the lesions were homogeneous on both precontrast images and postcontrast images. The signal intensity of the lymphoma was isointensity in 6 patients on T1 WI, and the others were hyperintensity. T2 WI showed a isointense signal in 1 cases and showed a slight hyperintense signal in 7 cases. After the administration of Gd - DTPA ,all the lesions enhanced moderately. Conclusion The understanding of MRI features of orbital lynlphoma is very useful for improving preoperative diagnostic accuray.
