摘　　要：目的评价大剂量醛氢叶酸加5氟尿嘧啶(5-Fu)持续48h滴注联合卡铂(CBP)方案治疗晚期食管癌的客观疗效及毒副反应,探索晚期食管癌较合理治疗方案。方法对入选的33例晚期食管癌病人采用大剂量醛氢叶酸(200mg/m2)+5-Fu(3.0g/m2)持续48h滴注联合CBP方案进行治疗,平均2.61个疗程。结果完全缓解(CR)3例,部份缓解(PR)12例,稳定(NC)17例,进展(PD)1例,有效率为45.45%。主要的毒副反应为恶心呕吐、脱发、骨髓抑制及口腔粘膜炎,经常规对症治疗后见好转。结论大剂量醛氢叶酸+5-Fu持续48h滴注联合卡铂方案治疗晚期食管癌有一定疗效,毒副反应较轻。Objective To assess the anti-tumor efficacy and safety of high-dose-folinic acid plus 5-fluorou- racil bolus and continuous infusion 48 hours combined with carboplatin in treating advanced esophageal carcinoma. Methods 33 patients with advanced esophageal carcinoma were treated with high-dose-folinic acid plus 5-fluorouracil bolus and continuous infusion 48 hours combined with carboplatin. Results There were 3 complete respone, 12 partial respone, 17 no changes and 1 progressive disease in this series with total response rate of 45. 45%. The main side effects include nausea and vomiting, alopecia, bone marrow suppression and mucositis. Other side effects were uncommon. All side effects were tolerable, after treatment the patients were seen making progress. Conclusion High-dose-folinic acid plus 5-fluorouracil bolus and continuous infusion 48 hours combined with carboplatin may be a safe and effective therapy for the patients with advanced esophageal carcinoma.
