急性胰腺炎患者血清粘附分子水平变化的临床意义

摘　　要：目的探讨细胞间粘附分子-1(sICAM-1)在急性胰腺炎(AP)发病中的作用,并研究ICAM-1的连续监测在AP的分类诊断中的意义。方法对于60例AP患者,其中急性水肿性胰腺炎(AEP)组36例和急性出血坏死性胰腺炎(AHNP)组24例,连续7天采用ELISA法测定其血清sI-CAM-1水平变化。结果AEP组和AHNP组患者血清sICAM-1差异有统计学意义(P<0.01),且两组患者血清sICAM-1均明显高于健康对照组(P<0.01):AEP患者sICAM-I浓度-时间曲线呈下降型,AHNP患者sICAM-1浓度-时间曲线呈递增型或复发型。结论sICAM-1在AP的发生、发展过程中发挥重要作用,sICAM-1升高的程度及方式与AP严重程度密切相关,对AP的分类诊断具有重要价值。Objective To investigate the function of serum soluble intercellular adhesion molecule-1 （sICAM-1） in acute panereatitis（AP）. To study the significance of continuous monitoring sICAM-1 in differential diagnosis of AP. Methods A total of 60 patients were investigated;36 patients with acute edematous panereatitis（ AEP）, and 24 with acute haemorragic necrotic pancreatitis（ AHNP）. The eoncentrations of sICAM-1 in the patients＇ serum were measured serially over a period of 7d by enzyme-linked immunosorbent assay（ELISA）. Results There was significant differenee between AEP group and AHNP group（P〈0. 01） ,the levels of ICAM-1 were higher in AEP group and AHNP groups than that in control group（P〈0.01） ;deereasing and peak sICAM-1 concentrations were found in the patients with AEP,sICAM-1 levels with an increasing or relapsing pattern were observed in the patients with AHNR Conclusion The sICAM-1 plays an important role in AP. Increasing degree and ehange mode are closely related to the severity of AP,and sICAM-1 is of great value in differential diagnosis of AP.
