摘  要:目的:探讨18F-双脱氧葡萄糖(FDG)PET显像探察胃肠道恶性肿瘤患者术后肿瘤复发和远处转移的应用价值。方法:40例胃肠道恶性肿瘤术后患者,其中食管癌8例,胃癌8例,结肠癌15例,直肠癌9例;进行18F-FDG显像。结果:18F-FDGPET显像诊断胃肠道恶性肿瘤术后复发和或转移的灵敏度93.8%、特异性87.5%、准确性92.5%;在18F-FDGPET显像真阳性30例中,4例为局部复发,9例为局部复发伴转移;17例为远处转移。在有复发的13例中,9例18F-FDGPET显像结果与X-CT结果一致,4例X-CT检查未见异常。在有转移的26例患者中,18F-FDGPET显像共捡出转移灶109处。结论:18F-FDGPET显像是检测胃肠道恶性肿瘤术后复发和转移的灵敏而有效的方法,值得推广应用。Objective:To investigate the clinical value of ^18F - fluorodeoxyglucose (^18F - FDG) PET imaging in detection of recurrence or metastasis in patients with postoperative gastrointestinal maglignant neoplasms. Methods: Forty patients of postoperative gastrointestinal maglignant neoplasms, including 8 esophageal cancer, 8 gastric cancer, 15 colon cancer, and 9 rectal cancer, underwent ^18SF - FDG PET imaging. Results: In all patients the sensitivity, specificity and accuracy of ISF - FDG PET imaging were 93.8% , 87.5% and 92.5% , respectively for diagnosis of recurrence and/or metastasis. There were 30 true positive patients detected by ^18F - FDG PET imaging; 4 patients with local recurrence only, 9 with local recurrence and remote metastasis, and 17 with remote metastasis only. Nine out of 13 patients with local recurrence showed the same results of CT and ^18F - FDG PET imaging; the remaining 4 patients showed normal CT but abnormal ^18F- FDG PET imaging. In 26 patients with remote metastasis, 109 metastatasis lesion were found. Conclusion: ^18F - FDG PET imaging is a useful and sensitive method in the detection of recurrence and metastasis in patients with postoperative gastrointestinal maglignant neoplasms.
