便携式化疗泵持续滴注治疗低度恶性淋巴瘤的临床研究

摘　　要：目的比较CHOP方案（环磷酰胺、阿霉素、长春新碱、强的松）常规给药与便携式化疗泵持续滴注治疗低度恶性淋巴瘤的近期疗效和毒性反应。方法患者随机分成2组，CHOP常规给药组28例，阿霉索40mg／m^2，长春新碱2mg；化疗泵组30例，阿霉素40mg／m^2＋长春新碱2mg／生理盐水200mL经便携式化疗泵持续滴注120h。两组环磷酰胺用越为750mg／m^2，强的松100mg口服，第1，5天；21天为1周期，共计4周期。结果常规给药组，CR4例（14．3％），PR6例（21．4％），有效率（RR）35．7％（10／28）；化疗泵组，CR8例（26．6％），PR11例（36．7％），有效率（RR）63．3％（19／30）。主要毒副反应为Ⅰ～Ⅱ度血液学毒性和消化道反应。结论CHOP方案中阿霉素和长春新碱经化疗泵持续滴注治疗低度恶性淋巴瘤安全、经济、方便、缓解率高，而且毒性反应低。Objective To compare effects and toxicities of CHOP（ cyclophosphamide, adriamycin, vincristine, prednisone）regimen by routine treatment or continuous infusion via portable chemotherapy pomp in the treatment of low-grade malignant lymphoma.Methods Fifty-eight cases were divided randomly into two groups. Routine treatment group（ n = 28） received adriamycin 40 mg/m^2 ; vincristine 2 mg. Chemotheraphy pump group（ n = 30） received adriamycin 40 mg/m^2 plus vincristine 2 mg in natriichloride 200 mL continuous infusion via portable chemotheraphy pump for 120 hours,The cyclophosphamide and prednisone dose of two groups were 750 mg/m^2 ; 100 mg, day 1 - 5. All groups repeated for 21 days;The total cycles were four. Results 4 of 28 patients in the routine treatment group were complete response（CR, 14.3 % ）,6 partial response（PR,21.4% ）. 8 of 30 patients in the chemotheraphy pump group were complete response（ CR, 26.6 % ）, 11 partial response（PR, 36.7 % ）. Major side-effects were Ⅰ - Ⅱ grade hematological and digestive toxicities. Conclusions Continuous infusion of adriamycin plus vincristine via portable chemotherapy pump in CHOP regimen was safe, economical, convenient in the treatment of low-grade malignant lymphoma, and the toxicities were slight.
