粘附分子变化与系统性红斑狼疮患者中的相互关系

摘　　要：目的了解各指标与系统性红宽狼疮（SLE）疾病的相互关系。方法对66例活动期SLE患者血清可溶性粘附分子1（sICAM-1）、sICAM-2、sICAM-3及血清抗核抗体（ANA）、补体C3、C4及红细胞沉降率（ESR）进行调查；对6例SLE患者，分别于临床活动期、恢复期、静止期三个不同阶段采集血标本进行各项指标动态观察，应用ELISA检测技术对各粘附因子、ANA进行调查；全自动双光径免疫浊度分析仪检测补体C3、C4；魏氏沉降法测定ESR。结果66例SLE患者各项指标均明显高于正常对照，两组间表现出差异有非常显著性（P均〈0．01）；动态观察结果表明，SLE患者随临床病情改善，各粘附因子及ESR、ANA水平，均呈逐渐下降的趋势，补体C3、C4逐渐上升。经相关性统计分析，只有sICAM-1与ANA、C4的水平变化分别表现出正相关与负相关（r=0．997，r=-0．993；P〈0．05）。结论粘附分子可能参与SLE疾病过程。Objective To detect the level of soluble intercellular adhesion molecules （sICAM）,ANA,ESR,C3,C4 on 66 systemic lupus erythematosus（SLE） patients. At the same time, dynamic results of these target were monitored on 6 SLE patients during the period of activation,restoration,inactivation. Then analyse the relation between the targets and the disease. Methods Use ELISA to examine sICAM - 1, sICAM - 2,sICAM - 3,ANA （anti nucleus antibody）,immage Immunochemistry System to check up C3, C4, Simultaneity measure ESR on 66 SLE patients and 12 normal cases. Results All results between SLE and normal cases were showed significant difference （all P 〈 0.01） ; the dynamic results showed the level of sICA M and A NA dechned gradually with the improvement of clinic symptom. But only sICA M - 1 level correlated well with ANA and C4 after statistical analysis （r= 0.997,r= -0.993; P 〈 0.05, P 〈 0.05）.Conclusions sICAM measurement may serve as an additional serologic marker of disease activity in patients with SLE.
