祛瘀消肿合剂选择性治疗自发性脑出血的临床疗效研究

摘　　要：目的探讨祛瘀消肿舍剂治疗自发性脑出血的临床疗效。方法选择164例自发性脑出血患者，并经影像学检查证实，且均为急性期。随机分为治疗组84例，对照组80例，进行治疗观察。结果治疗组除6例因继续脑出血患者病情加重中转手术外，其余给予祛瘀消肿合剂配合西药治疗，3-5d临床症状开始好转，治疗21-42d，平均3l，5d后复查颅脑CT，90％患者出血灶完全吸收，10％患者出血灶液化呈囊样变，80％患者肢体功能恢复快。对照组除8例因继续脑出血患者中转手术外，其余西药治疗，临床症状消失时间明显延长，一般3-4w方渐消失，出血灶吸收时间亦较长，一般在40-60d，平均50d，在此期间复查颅脑CT，仅80％患者出血灶完全吸收，20％患者出血灶液化呈囊样变，患者肢体功能恢复慢。结论科学稳妥地利用祛瘀消肿合剂，治疗自发性脑出血，使多数患者免除手术风险及痛苦，缩短了病程，取得了较满意的效果，有很高的临床推广价值。Objective To explore the clinical effect of Detumescence Mixture in treating spontaneous cerebral hemorrhage. Methods 164 patients diagnosed with CT or MRI to have spontaneous cerebral hemorrhage were divided into control group （84） and treatment group （80） randomly.Results In the treatment group, all the patients have early improvement after treatment with Detumescence Mixture and standard pharmacological treatment for 3 - 5 days, except that six patients received operation after deterioration. After being treated for 21 - 42 days, the blood was absorbed completely in 90 percent of patients. In 10 percent of patients, the blood in hemorrhage focus was liquefied and became cysts. For 80 percent of patients, the motor functions had been improved more quickly compared to the patients among control group. In the control group, all the patients were treated with standard medicine except that 8 patients received operations ＇after continuous cerebral hemorrhage. Compared to the treatment group, it took longer time for patients to recover the motor function （3 - 4 weeks） and absorb the blood of hemorrhage focus （40 - 60 days）. According to CT resuhs, the blood of hemorrhage focus in 80% patients from control group were completely absorbed and there were cysts formation in 20 percent of patients during the period. Conclusions Detumescence Mixture can be used to treat spontaneous cerebral hemorrhage scientifically and carefully, which can exempt the needs of operation in part and reduce suffering of patients. Therefore Detumescence Mixture can reduce the time for rehabilitation of patients with cerebral hemorrhage and it is worth further clinical trial.
