垂体催乳素瘤与高催乳素血症的临床研究

摘　　要：将垂体催乳素瘤和高催乳素血症41例,经测定血清催乳素(PRL)值.结合CT及/或MRI检查后分为垂体巨腺瘤组、垂体微腺瘤组和特发性高催乳素血症组.8例巨腺癌及4例微腺瘤患者曾被施行手术或加放疗.全部病例口服溴隐亭并长期随访(平均31.1±19.1月).结果:月经恢复率为86.5%,妊娠率达70.8%.微腺瘤组和特发性高催乳素血症组的疗效比巨腺瘤组显著(P<0.05).单用溴隐亭者性腺功能恢复更好(P<0.05),部分病例还有肿瘤缩小的征象.研究结果提示服用溴隐亭是无压迫症状的垂体催乳素腺瘤的首选治疗手段.Management of 41 female patients with prolactinomas and hyperprolactinemia in our department since June 1988 were studied. The patients were classified into 3 groups:(1) 15macroademas ; ( 2 ) 18 microadenomas ; (3) 8 hyperprolactinemia. Twelve patients had under gone operations with or without radiotherapy. All the patients received oral bromocriptine (Br) and had a long-term follow-up(average term 31. 1 - 1 9. 1 month). As a resut 86.5% of the patients resumed normal menses and the pregnancy rate reached 70. 8%, Compared the treatment efficacy of the microadenomas nad the idiopathic hyperprolactinemia with that of the macroadenoma .the former showed greater efficacy (P<0. 05). Compared the primary Br therapy with the Br therapy after the surgical operation and radiotherapy. the former had a better gonadal function (P<0. 05). Our study provides a clear evidence that the bromocriptin offers a superior choice of treatment for patients requiring fertility with prolactinoma without oppression symptoms.
