摘  要:目的了解宫颈癌中hMLH1、P27、P53表达情况及其与宫颈癌发生和发展的关系。方法采用免疫组织化学技术SP法检测临床资料较完整的72例子宫颈鳞癌和31例慢性宫颈炎组织hMLH1、P27、P53蛋白表达。结果(1)hMLH1在宫颈炎及宫颈癌组的表达率分别为61.3%和58.3%,差异无显著性(P>0.05);hM-LH1在宫颈癌各期中表达率分别为I期60.0%,Ⅱ期52.6%,Ⅲ期33.3%,差异无显著性(P>0.05);hMLH1表达率与宫颈癌分级无关(P>0.05);(2)P27在宫颈炎及宫颈癌组的表达率分别为74.2%%和76.4%,差异无显著性(P>0.05);P27的表达率与宫颈癌分级和分期无关(P>0.05);(3)P53在宫颈炎及宫颈癌组的表达率分别为32.3%和65.3%,差异有显著性(P<0.05);P53在宫颈癌各级中的表达率分别为I级36.4%,Ⅱ级73.0%,Ⅲ级67.9%,Ⅰ级与Ⅱ、Ⅲ级的表达率差异有显著性(P<0.05);P53表达率与宫颈癌分期无关(P<0.05);(4)hMLH1表达阳性与阴性的宫颈癌中P27的表达率为94.6%和60.0%,差异有显著性(P<0.05);P53的表达率为62.8%和62.1%,差异无显著性(P>0.05)。结论P53基因突变与宫颈癌的发生和组织分化程度有关;hMLH1与P27表达缺失和宫颈癌发生的关系尚不确切。hMLH1可能使宫颈癌中P27表达上调,而对p53基因突变累积无明显抑制作用。Objective To investigate the significance of hMLH1,P27 and P53 in expressions in the pathogenesis and development of cervical carcinoma. Method hMLH1 ,P27 and P53 protein expressions were detected by immunohistochemieal analyses of formalin-fixed, paraffin-embedded samples from 31 chronic cervical inflammation (CCI) and 72 squamous cervical earcinoma(SCC). Results (1) The hMLH1 expression rates were 61.3% and 58.3% in CCI and SCC respectively (P〉0.05) ;60.0%,52.6% and 33.3% in stage Ⅰ,stage Ⅱ and stage Ⅲ of SCC respectively, which had no significant difference (P〉0.05);hMLH1 protein expression rates were not associated with the grade of SCC. (2)The P27 expression rates were 74.2%and 76.4% in CCI and SCC rcspectively(P〉 0.05); P27 protein expression rates were not associated with the grade and stage of SCC. (3)The P53 expression rates were 32.3% and 65.3% in CCI and SCC respectively(P〈0.05) ;36.4% ,73.0% and 67.9% in grade Ⅰ,grade Ⅱ and grade Ⅲ of SCC respectively,the expression rates between grade Ⅰ and grade Ⅱ , Ⅲ were significantly different;P53 protien expression rates were not associated with the stage of SCC. (4)In hMLH1 (+) and hMLH1 (-) groups of SCC,P27 protein s expression rates were 94. 6% and 60. 0%, respectively(P〈0.05) ; P53 protein" s expression rates were 62.8% and 62.1 % respectively(P〉0.05). Conclusions P53 gene mutation plays an important role in cervical carcinogenesis and differentiation. The relationship between defection of hMLH1 and SCC is still not clear. The study suggests that the defection of hMLH1 may be associated with the development of SCC,which need to be further studied, hMLH1 and P27's overexpression in SCC may be a kind of accommodation, hMLHI had no inhibit effect on P53.
