再灌注期间联合应用牛磺酸与二甲基氨氯吡咪(DMA)对心肌损伤的保护作用  被引量:1

摘  要:目的进一步探讨心肌缺血-再灌注损伤发病机制及寻求有效的防治药物。方法采用Langendorff法建立离体大鼠心肌缺血-再灌注模型,全心缺血45 min后,再灌注30 min,在再灌注期间给予台氏液灌流作为对照;而药物干预分别为再灌全过程给予牛磺酸,再灌早期给予DMA,两者联合应用。观察药物对离体灌流心脏血流动力学指标的作用,同时观察再灌注期间心律失常的发生情况以及冠脉流出液中肌酸激酶的活性变化。结果再灌注期间全程给予20 mmol/L牛磺酸对于改善血流动力学指标LVSP-LVDP,+dp/dtmax,-dp/dtmax有一定的作用,但较对照组不具统计学意义。再灌注早期给予20μmol/L DMA后上述血流动力学指标较对照组有显著改善。牛磺酸组和DMA组再灌注时心律失常的发生率均较对照组降低,且流出液中肌酸激酶(CK)的活性下降。联合使用二者显示协同效应。结论再灌注期间联合使用牛磺酸和DMA能从很大程度上减轻心肌缺血-再灌注损伤。Objective To explore deeply the mechanism of myocardial ischemia reperfusion injury and search effective preventative drugs. Methods The myocardial ischernia reperfusion model was established by Langendorff system after 45-min ischemia. The isolated rat heart was reperfused 30 min. Taurine was added to the perfusate during all the reperfusion period and DMA only during the early stage of reperfusion. Effects of the drugs on hemodynamics indexes and the incidence rate of arrhythmia during reperfusion were observed. Effects of the drugs tested on creatine kinase (CK) activity in coronary effluent were tested. Results Taurine (20 mmol/L) could improve the hemodynamics indexes including LVSP- LVI)P, + dp/dtmax, - dp/dtmax,but it had no statistical significance compared with the controls. DMA (20 nmol/L) can markedly improve the above hernodyanamies indexes. The incidence rate of arrhythmia was depressed in both taurine and DMA groups compared with the control during reperfuslon period, and the CK activity in the coronary effluent was also reduced. Combination of taurine and DMA had co-effect. Conclusion Combination of taurine and DMA during the reperfusion period can significantly relieve the myocardial ischernia reperfusion injury.
