宫颈癌凋亡抑制因子表达与HPV_(16/18)感染相关性

摘　　要：目的研究凋亡抑制因子B细胞淋巴瘤-2(Bcl-2)和生存蛋白(Survivin)在宫颈癌组织中的表达及其与人乳头瘤病毒(HPV)感染的相关性。方法采用免疫组织化学方法,对宫颈癌、宫颈上皮内瘤样病变(CIN)和正常宫颈组织的石蜡标本进行Bcl-2和生存蛋白的检测,同时采用DNA缺口末端标记技术(TUNEL法)和PCR技术检测细胞凋亡指数(AI)和HPV16/18感染情况。结果宫颈癌组织、CIN和正常宫颈3种组织中,Bcl-2、生存蛋白表达阳性率和AI的差异均有统计学意义(均P<0.01),且随着临床分期病理分级的升高阳性率差异有统计学意义,其表达率与淋巴结转移有关(P<0.05)。在正常宫颈、CIN和宫颈癌中的HPV16/18阳性率逐渐升高,差异有统计学意义,但与临床分期、病理分级、淋巴转移无关。Bcl-2、生存蛋白和AI均与HPV16/18感染有相关性。结论宫颈癌组织中的Bcl-2、生存蛋白的异常表达与HPV16/18感染有关,与宫颈癌的恶变程度有关,可作为宫颈癌筛查预后判断的有利指标。Objective To investigate relationship between expression of inhibitors of apoptosis proteins（IAPs）, such as Bd-2 and Survivn, and HPV16/18 infection in uterine cervical carcinogenesis, Methods Formalin-fixed, paraffin-embedded samples from cervical carcinomas, cervical intraepithelial neoplasia（CIN）and normal eevical tissues were studied, Using streptavidin-biotin roxidase（S-P）immunohistochemical technique, the authors examined the expression of Bcl-2 and Survivin in these samples. The number of apoptosis cells was assessed in situ by TdT-mediated dUTP-biotin end labeling （TUNEL） method. The infection of HPV type 16, 18 DNA were determined by PCR. Results There were significant differences for Bcl-2 and Survivn and AI were corrlated with tumor grades, clinical stages and lymph node metastasis. The positive rate of HPV16/18 was significant difference between cervical carcinomas, CIN and normal tissues and was not associated with tumor grades, clinical stages and lymph node metastasis. The infection of HPV16/18 was associated with the expression of Bcl-2, Survivin and AI, respectively. Conclusion The abnormal expression of Bcl-2 and Survivin are associated with the infection of HPV16/18, They possibly become useful indexes for the primary screening and prognosis of cervical carcinomas.
