选择性低温对犬脑缺血性损害的保护作用初步探讨

摘  要:目的:建立经脑室低温液灌注选择性脑低温模型,并观察其对犬脑缺血性损害的保护作用。方法:犬全脑缺血30 min恢复血液灌注后,立即或半小时后经一侧侧脑室持续低温等渗盐水灌注(20℃),对侧侧脑室引流,4 h后自然复温,作脑组织染色及干湿比重测定。结果:即时和延时低温组平均脑温降低至亚低温水平,灌注侧需30 min,引流侧需45 min;对照组脑温没有明显变化;各组动物的肛温没有明显下降。3组动物脑组织干湿重比差异有统计学意义(P<0.01),海马CA1区组织病理改变也存在差异,组织学损害和脑水肿程度以对照组最重,即时低温组最轻。结论:经脑室低温液持续灌注可实现选择性脑低温,对犬脑受缺血性损害的神经元有保护作用,其效果与低温开始的时机有关。Objective: To construct canine selective brain hypothermia model and evaluate its effects on globe brain ischemic model. Methods: The cerebral bllld supply of all canines was interrupted for 30 minutes, then were recovered. Constantly perfusing isotonic nitric chloride (at 20℃) into one of their lateral ventricles, this was initiated whether promptly at the beginning of blood supply recovery or 30 minutes after blood supply recovered, and draining the solution from the other side at the same time. Then sustain the perfusing for 4 hours and lower the brain temperature to the level of mild hypothermia. Results: As for two control groups, it took 30 minutes to lower the brain temperature of pea'fusing side to the level of mild hypothermia, 45 minutes to lower the other side, while rectal temperature was nearly unchanged. Water content and pathologic changes of hippccampus CA1 area among groups are significantly different. Conclusion: The hypothermia model is useful which can protect neuronal from ischemic damage. The effects have relation with the initiating time after cerebral ischemia.
