摘  要:目的评价术前应用奥曲肽对垂体生长激素腺瘤瘤体中Ⅰ型、Ⅲ型胶原含量的影响。方法25例肢端肥大症病例随机分为两组,治疗组13例,对照组12例。治疗组术前用奥曲肽,2周后手术,对照组术前未用奥曲肽直接行经蝶手术。肿瘤标本行Ⅰ型、Ⅲ型胶原免疫组织化学染色,计算Ⅰ型、Ⅲ型胶原阳性面积百分比,比较两组术后Ⅰ型、Ⅲ型胶原含量。结果治疗组和对照组中,Ⅰ型胶原阳性面积分别为12.75%±5.01%和36.86%±11.27%(P<0.05),Ⅲ型胶原阳性面积为11.99%±4.13%和13.12%±5.13%(P>0.05)。结论术前应用奥曲肽,使瘤体中Ⅰ型胶原含量减少,进而可能软化瘤体,为经蝶手术治疗创造有利条件。Objective To evaluate the effects on content of type Ⅰ and Ⅲ collegen of pituitary hormone adenomas during preoperative usr of octreotide. Methods From January 2004 to March 2006, 25 cases with pitaitary growth hormone adenomas were randonly divided into 2 groups. The therapeutic group was in 13 cases and control group was 12 cases. The octreotide was used in the therapeutic group before operation and the transsphenoidal operation was performed after 2 weeks, in the control group, the transsphenoidal operation was performed without use octereotide before operation. The type Ⅰ and Ⅲ collegen immunohisto chemical staining were done in tumor specimen. The positive area percentage (PAP) of type Ⅰ and Ⅲ collegen were calculated and content of type Ⅰ and Ⅲ collegen were compared 'after operation. Results In the therapeutic group and control group, the PAP of type Ⅰ collegen was 12.72%± 5.01 and 36.86%±11.27 respectively (P〈0.05) and PAP of type m collegen was 11.99%±4.13% and 13.12%±5.13% respectively (P〈0.05). Conclusion When preoperative use of octreotide, it can reduce content of type Ⅰ collagen of tumor and soften tumor tisue. So it would offer convenient condition of transsphenoidal surgery.
