摘  要:目的研究99mTc-HL91乏氧显像与99mTc-MIBI亲肿瘤显像对肺癌诊断的临床价值。方法对25例肺部原发性恶性肿瘤患者和11例对照者行99mTc-HL91乏氧显像和99mTc-MIBI亲肿瘤显像。经99mTc-HL91断层显像后1d行99mTc-MIBI断层显像,并对显像阳性患者利用感兴趣区(ROI)技术分别勾画肿瘤(T)与对侧相应部位(N),计算T/N比值。结果99mTc-HL91肺部病灶显影灵敏度、特异性和准确度分别为84·0%(21/25),90·9%(10/11),86·1%(31/36),99mTc-MIBI显像76·0%(19/25);90·9%(10/11);80·5%(29/36)。随访好转组与死亡组99mTc-HL91乏氧显像及99mTc-MIBI显像肺部病灶T/N比值分别为2·17±0·41、3·12±0·72(t=3·817,P<0·01)和3·43±0·62、1·99±0·74(t=3·010,P<0·05)。结论99mTc-HL91乏氧显像与99mTc-MIBI亲肿瘤显像诊断肺癌具有较高的临床应用价值,两者均能对患者预后进行初步判断。Objective To study the clinical evaluation of ^99mTc-HL91 hypoxic imaging and ^99mTc-MIBI imaging for diagnosing lung cancer. Methods ^99mTc-HL91 hypoxic imaging and ^99mTc- MIBI imaging were performed in 25 patients with primary lung malignant tumors and 11 control subjects. ^99mTc-MIBI SPECT tomograph imaging was done 1 day after ^99mTc-HL91 tomograph imaging. The regions of interesting(ROI) were drawn in the tumor and contralateral position to calculate the radioactivity ratios of tumor to normal (T/N). Results The sensitivity, specificity and accuracy of ^99mTc-HL91 imaging and ^99mTc-MIBI imaging on the lesion of lung tumor were 84.0%(21/25), 90. 9%(10/11)and 86.1% (31/36), and 76.0% (19/25), 90. 9% (10/11) and 80.5%(29/36), respectively. T/N ratios of 99mTc-HL91 imaging and 99mTc-MIBIimaging on the lesion-to-normal lung tissue in better groups and death groups were 2.17_--1-0.41 and 3.12~0. 72 (P〈O. 01), and 3.43-t- 0. 62 and 1.99=t=0.74 (P〈0. 05),respectively. Conclusion 99mTc-HL91 hypoxic imaging and ^99mTc- MIBI imaging are useful in diagnosing lung cancer and predicting prognosis.
