内镜下氩离子凝固术治疗非食管静脉曲张性上消化道出血46例分析

摘　　要：目的:探讨内镜下氩离子凝固术(argon plasma coagulation,APC)治疗上消化道出血的临床应用价值。方法:采用山东济南玉华电器有限公司生产的AG-9800 II型内镜专用多功能氩离子凝固仪(又称氩气刀),对非食管静脉曲张性上消化道出血患者进行治疗。结果:经氩离子凝固术治疗后,46例上消化道出血患者中,44例患者未再出血,2例患者再出血,总有效率95.2%。其中28例十二指肠球部溃疡和2例D ieulafoy病患者未发生再出血,12例胃溃疡患者有1例发生再出血,4例贲门撕裂综合征患者中发生再出血1例。结论:内镜下氩离子凝固术治疗非食管静脉曲张性上消化道出血具有疗效确切,安全快速,并发症少,患者痛苦小等特点,临床应用价值较高。Objective： To explore the clinical application of endoscopic APC （argon plasma coagulation ） in the treatment of patients with upper gastrointestinal hemorrhage. Method： AG - 9800Ⅱ Argon plasma coagulator produced by Shandong Yuhua Electrice Co. , Ltd is used to treat the patients with non - variceal upper gastrointestinal hemorrhage. Result： After the treatment with APC, of the 46 patients with upper gastrointestinal hemorrhage, 44 patients cease to hemorrhage and 2 patients rehemorrhage. The effective rate was 95.2%. No hemorrhaging happens to the 28 patients with duodenal ulcer and 2 patients with Dieulafoy. Rehemorrhaging happens to 1 of the 12 patients with stomach ulcer and 1 of the 4 patients with MalloryWeiss syndrome. Conclusion： Endoscopic argon plasma coagulation for the treatment of patients with upper gastrointestinal hemorrhage is safe and quick, has definite effectiveness, and leads to less complications and less pain. It has a high value of clinical application.
