不同部位牙冠延长术的临床疗效研究

摘　　要：目的:观察牙体缺损不同部位牙冠延长术后的疗效。方法:对38颗牙体缺损达龈下的患牙行牙冠延长术,以恢复患牙的生物学宽度。术中将31例缺损位于龈下≤3mm的患牙的牙槽嵴顶降至断缘根方3mm,4例缺损位于龈下≥4mm仅进行骨缘的修整,≥5mm的仅做牙龈的部分切除;并分别于术后1周、2周、4周及6周以及永久修复3个月、6个月进行临床牙周检查。结果:牙体缺损位于龈下≤3mm的31例在术后6周获得了比较好的临床结果,修复6个月后牙周组织保持健康;牙体缺损位于龈下≥4mm的7例患牙在术后6周及修复6个月后牙龈仍呈现炎症。结论:当牙体缺损断端位于龈下≤3mm,手术去除牙槽骨高度达3mm时,冠延长术能获得比较好的效果。Objective：To observe the clinical results of the post surgical crown lengthening for different cases of teeth with subgingival destruction.Methods：On 38 teeth due to various crown damages or vertical fractures in subgingival lesions were performed with surgical crown lengthening in order to achieve biologic width.The distance between the margin of tooth and bone crest was gained at least 3mm on 31 teeth which damages subgingivally less than 3mm,the other 7 teeth only performed gingivectomy.All parameters included in the periodontal health were examined 1,2,4,6 weeks follwing surgery.Results：The good periodontal health can be gained on 31 teeth after surgery in 6 weeks and 6 month,respectively,while 7 teeth were not so good. Conclusion：The better result can be achived if the subgingival lesion of tooth is less than 3mm and ideal exposure of tooth margin at least 3mm follwing surgical crown lengthening.
