摘 要:目的运用相关的造影数据分析软件,定量分析超声造影诊断肝肿瘤的准确性。方法运用QLAB—ROI“时间-强度”曲线分析软件对53例肝局灶性病变超声造影图像,进行始增时间、峰值时间、峰值强度、1/2消退时间定量分析。结果(1)肝恶性与良性肿瘤比较:始增时间、峰值时间、峰值强度均无统计学意义(p>0.05)。1/2消退时间恶性肿瘤明显早于良性肿瘤(p<0.001)。(2)原发性肝癌与转移性肝癌比较:始增时间、峰值强度无统计学差别(p>0.05)。原发性肝癌的峰值时间早于转移性肝癌(p<0.05),1/2(?)退时间晚于转移性肝癌(p<0.05)。(3)本组37例良性肿瘤可分为早期回声增强组与回声不增强组。二组在始增时间、峰值时间、峰值强度比较均无统计学意义(p>0.05)。1/2消退时间,回声增强组明显晚于回声不增强组(p<0.01)、(4)肝脏恶性肿瘤或良性肿瘤,其肿瘤结节与周边组织时间-强度曲线参数对照,无论在始增时间、峰值时间、峰值强度、1/2消退时间上,均有显著意义。结论超声造影技术对肝局灶性病变的诊断和鉴别诊断有重要的临床应用价值。结合“时间-强度”曲线参数定量分析软件,对进一步提高肝肿瘤诊断的准确性有较大帮助。Objective To analyze the accuracy of contrast-enhanced uhrasonography with QLAB-ROI software in diagnosis of focal liver lesions (FLL). Methods Fifty-three cases of FLL were analyzed by contrast-enhanced uhrasonography with QLAB-ROI time-intensity curve software. The study involved four parameters: enhancement beginning time, peak time, peak intensity and half falling time. Results There was no significance of enhancement beginning time, peak time and peak intensity between benign and malignant FLLs(p〉0.05), But the half falling time in the malignant FLLs was earlier than the benign ones. (p〈0.001). There was no significance of enhancement beginning time, peak intensity between primary hepatocelluar carcinoma (PHCC) and metastasis (p〉0.05), but the peak time in PHCC was earlier than metastasis (p〈0.05), and was later in the half falling time (p〈0.05). There was also no significance of enhancement beginning time, peak time, peak intensity between the early-enhanced benign FLLs and the early-no-enhanced group(p〉0.05), and the half falling time was later than the other(p〈0.01). There was statistical significance of enhancement beginning time, peak time, peak intensity and half falling time between the benign and malignant FLLs compared with liver parenchyma. Conclusion Real time contrast-enhanced ultrasonography with QLAB-ROR analysis software is a promising method in the differential diagnosis of liver neoplasms.
