摘 要:目的评估细胞因子白细胞介素-2(interleukin-2,IL-2)及其受体(IL-2 receptor,IL-2R)在造血干细胞移植期间的变化和临床价值。方法用ELISA法检测了34例造血干细胞移植患者在移植前、预处理后、骨髓抑制期、感染期、造血恢复后2个月以及移植物抗宿主病(graft versus host disease,GVHD)发生时血清IL-2和IL-2R的水平。结果在发生细菌感染的患者中,IL-2和IL-2R分别为233.4±103.6 pg/mL和357.4±159.2 pmol/L,显著高于未发生细菌感染组(169.7±37.3 pg/mL和257.6±48.8 pmol/L,P=0.0490和0.0453)和正常对照组(167.8±38.6 pg/mL和249.7±48.1pmol/L,P=0.0111和0.0062)。同时,两者在巨细胞病毒(Cytomegavirus,CMV)感染的患者中(291.4±51.7pg/mL和326.5±108.6 pmol/L)亦显著高于CMV阴性组(187.2±69.7pg/mL和225.3±46.1 pmol/L,P均=0.0005)和正常对照组(P=0.0002和0.0141)。此外,患者在发生GVHD时IL-2和IL-2R(328.6±141.7)pg/mL和321.8±49.2 pmol/L)较移植前(201.4±83.4 pg/mL和250.8±39.7 pmol/L)均显著增高(P=0.0055和0.0105)。结论细胞因子IL-2及其受体在感染的发生和GVHD的发病中起重要的正向调节作用,定期检测患者血清IL-2和IL-2R水平有助于预测感染和GVHD的发生。Objective To evaluate the clinical significance of serum interleukin-2 (IL-2) and its receptor (IL-2 receptor,IL-2R) in patients received hematopoietic stem cell transplantation. Methods Using ELISA, serum IL-2 and IL-2R levels were measured before transplantation, after preparation regimen, at the period of myelo-suppression, infection, bone marrow recovery, and at the presence of graft versus host disease (GVHD).Results In patients with bacterial infection,IL-2 and IL-2R were respectively 233.4 ± 103.6 pg/mL and 357.4 ± 159.2 pmol/L, significantly higher than those without bacterial infection (169.7 ± 37.3 pg/mL and 257.6 ± 48. 8 pmol/L, P = 0.0490 and 0.0453) and the normal control (167.8±38.6 pg/mL and 249.7 ±48.1 pmol/L, P = 0.0111 and 0. 0062) .Meanwhile,they also significantly increased in patients with cytomegavirus (CMV) infection (291.4 ± 51.7 pg/mL and 326. 5 ± 108.6 pmol/L) ,comparing to those CMV-negative cases (187.2 ± 69.7 pg/mL and 225.3 ± 46.1 pmol/L,P both = 0.0005), and the normal control ( P =0.0002 and 0.0141 ). When GVHD happened, IL-2 and IL-2R (328.6 ± 141.7 pg/mL and 321.8 ± 49. 2 pmol/L) were significantly higher than before transplantation (201.4 ± 83.4 pg/mL and 250.8 ± 39.7 pmol/L, P = 0.0055 and 0.0105).Conclusin Conclusion: Cytokine IL-2 and its receptor may play important roles in the pathogenesis of infection and GVHD. Assessment of serum IL-2 and IL-2R levels during hematipoietic stem cell transplantation can provide predictive indicator for infection and GVHD.
