摘　　要：目的探讨非小细胞肺癌(NSCLC)患者化疗前后外周血中肿瘤标记物癌胚抗原(CEA)、细胞角蛋白19片断21-1(CYFRA21-1)水平的检测及其意义。方法采用放射免疫技术对116例非小细胞肺癌患者化学治疗前后血清肿瘤标记物CEA、CYFRA21-1水平进行检测,并根据化疗前后肺部CT影像学改变进行分析。结果CEA、CYFRA21-1水平在NSCLC化疗前后有明显变化(P<0.05);肺癌组化疗有效者(CR+PR)2种肿瘤标记物水平显著下降(P<0.05),病情稳定(NC)和进展者(PD)二者水平无变化(P>0.05);CEA对于肺腺癌,CYFRA21-1对于肺鳞癌化疗前效果的评定具有一定的价值。结论检测CEA、CYFRA21-1水平有助于非小细胞肺癌化疗后疗效的判断。Objective To investigate the value of combined detection of serum CEA and CYFRA21 - 1 markers in the patients with NSCLC before and after chemotherapy. Methods Using immunoradiometrie assey, the serum CEA and CYFRA21 - 1 markers were measured in 116 eases of NSCLC before and after chemotherapy, while the changes of Computed Tomography（ CT ） images were analysed. Results After chemotherapy, the levels of CEA and CYFRA21 - 1 in patients with NSCLC were decreased significantly than those before chemotherapy（ P 〈 0.05 ）. The levels of CEA and CYFRA21 - 1 were decreased significantly in the CR and PR patients （ P 〈 0.05 ）, but increased or not changed in NC and PD ones （ P 〉 0.05 ） . Before and after chemotherapy, the changes of CEA were valuable in adenoeareinoma patients, and so did CYFRA21 - 1 in squamous carcinoma ones. Conclusion Combined detection of serum CEA and CYFRA21 - 1 markers is value in monitoring and evaluating therapeutic efficacy in NSCLC patients.
