HPV感染和P16^(INK4A)蛋白表达与宫颈癌关系的研究

摘　　要：目的探讨HPV(16/18型)感染和P16INK4A蛋白表达在人类宫颈鳞癌(SCC)及其癌前病变(CIN)中的关系。方法利用原位杂交和免疫组化方法研究了80例慢性宫颈炎及宫颈上皮内瘤变组织和宫颈鳞癌组织中HPV感染以及P16INK4A表达的情况。结果(1)与慢性宫颈炎相比,CINⅡ级、CINⅢ级、浸润癌HPV16、18杂交信号阳性率显著增高(P<0.01);(2)宫颈鳞癌组织、CINⅠ级、CINⅡ级、Ⅲ级及慢性宫颈炎标本中P16INK4A阳性率分别为100%、20.0%、46.7%、100.0%和10.0%;(3)在宫颈鳞癌及CINHPV16、18感染的标本中P16INK4A表达均是阳性。结论宫颈鳞癌的形成与HPV感染、P16INK4A过表达是呈正相关关系,P16INK4A可作为CIN的标志物对宫颈癌筛查和预防有重要意义。Objective To investigate the relationship between HPV infection and the expression of P16^INK4A in cervical intraepithelial neoplasia（CIN） and cervical squaous cell carcinomas（SCC）. Methods In situhybridization （ISH） and immunohistochemical method were used to detect the expression d HPV and P16^INK4A in all 80 cases of chronic cervicitis, CIN and SCC. Results （1）The positive rate of HPV hybrid signals in CIN Ⅱ, CIN Ⅲ and SCC were all significantly higher than in chronic cervicitis （ P 〈 0.01）. （2） Positive expression rotes of P16^INK4A in cervical squamous carcinoma, CIN Ⅰ , CIN Ⅱ, CIN Ⅲ and chronic cervicitis were 100 %, 20.0 %, 46.7 %, 100.0 % and 10.0 %. （3）P16^INK4A positive was found in which HPV16,18 infection of cervical squamous carcinoma and CIN. Conclusion There showed positive correlation between very high expression of P16^INK4A and HPV infection in CIN and SCC. P16^INK4A can be an indicator in CIN, Which has very significance in preventing the P16^INK4A infection and can work efficiently in SCC.
