重组人粒-巨噬细胞集落刺激因子治疗放射性口腔黏膜炎疗效分析

摘　　要：目的观察重组人粒-巨噬细胞集落刺激因子(rhGM-CSF)局部应用对放射性口腔黏膜炎的疗效。方法56例头颈肿瘤患者随机分为两组,治疗组32例,使用rhGM-CSF漱口液含漱(rhGM-CSF100μg冻干粉末用1ml蒸馏水溶解后加入200ml生理盐水中),5次/d;对照组24例,常规处理。根据RTOG急性放射性黏膜炎的分级标准进行临床评价。结果治疗组Ⅲ、Ⅳ度急性放射性口腔黏膜损伤发生率18.8%(6/32),明显低于对照组(75.0%,18/24;P=0.0001)。rhGM-CSF治疗放射性口腔黏膜损伤的有效率较对照组显著提高(87.5%对45.8%,P=0.0019),口腔黏膜溃疡平均愈合时间5d。结论rhGM-CSF局部应用,对放射性口腔黏膜损伤有很好的治疗作用,可刺激表皮生长因子活性,促进溃疡早日愈合。Objective To evaluate the efficacy of rhGM-CSF in treatment of acute oral mucositis induced by radiation. Methods Fifty-six head and neck cancer patients were randomly divided into 2 groups. The treatment group was treated with rhGM-CSF,5 times perday. The control group was treated with routine drugs. The evaluation of acute radiation mucositis was done according to the systems proposed by RTOG. Resuits The results showed that the incidences of grades＋ Ⅲ and -Ⅳ acute radiation mucositis in the treatment group,were lower than those in the control group. The response rate of acute radiation mucositis in the treatment group was also better than that in the control group（87.5% vs 45.8% ,P =0.0019）. The healing time of acute radiation mucositis was 5d in the treatment group. Conclusion rhGM-CSF is a very effective drug for acute radiation mucositis. When topically administered to the ulcer surface,it can stimulate the activity of epidermal growth factor and accelerate the healing of ulcers.
