大肠癌合并糖尿病40例围手术期处理

摘　　要：目的探讨大肠癌合并糖尿病的围手术期处理措施。方法对予以手术治疗的40例大肠癌合并糖尿病患者作回顾性分析。结果本组40例均为2型糖尿病、予以手术治疗。术后并发症发生率为:切口感染10%(4/40)、吻合口瘘5%(2/40)、肺部感染17.5%(7/40)、腹腔感染10%(4/40),无酮症酸中毒及手术死亡病例。结论大肠癌合并糖尿病只要围手术期严格控制血糖,合理应用胰岛素,可达到理想的外科治疗效果。Objective To investigate the methods of the perioperative period therapy on large bowel cancer associatod with diabetes meUitus-following 40 cases. Methods 40 patients with large bowel associated with diabetes mellitus were analyzed retrospectively. Results The incidence of postoperative complications was as follows ： wound infection in 4 cases（ 10% ）;intra-abdominal abscess in 4 cases, pneumonia in 5 cases and anastomotic leak in 2 cases. There were no postoperative diabetic ketoacidosis and operative death. Conclusion If the patients＇ blood-glucose concentration perioperatively could be well controlled and insulin be utilized reasonably, the effect of surgical treatment on large bowel cancer associated with diabetes mellitus would be well gratified.
