摘  要:目的:阐述影响纤维蛋白代谢的纤溶酶原激活物和纤溶酶原激活物抑制因子,分析细胞外基质成分和结构的改变对瘢痕的影响。资料来源:应用计算机检索中国期刊全文数据库1979-01/2006-05有关瘢痕病理发生机制的文章,检索词为“瘢痕,纤维蛋白,纤溶酶原激活物,纤溶酶原激活物抑制因子”,并限定文章语言种类为中文。同时检索Pubmed所有有关瘢痕病理发生机制的文献,检索词为“scar,fibrin,urokinase-type plasminogen activator,plasminogen activator inhibitor-1”,并限定文章语言种类为English。资料选择:对资料进行初审,选取包括与纤维蛋白和瘢痕相关文献,开始查找全文。纳入标准:与瘢痕病理发生机制相关文献。排除标准:临床治疗。资料提炼:共检索到关于纤维蛋白和瘢痕的文献692篇,其中英文文献217篇,纳入16篇符合标准的文献进行综述。资料综合:病理性瘢痕与创伤密切相关,它的发生可能是由于纤维蛋白异常沉积延长了急性反应期造成的。瘢痕组织中存在组织纤维蛋白溶解系统,主要由尿激酶型纤溶酶原激活物和纤溶酶原激活物抑制因子1构成,尿激酶型纤溶酶原激活物-纤溶酶原激活物抑制因子1的失衡致使纤维蛋白不能完全降解。组织纤维蛋白溶解系统可能在病理性瘢痕的病理发生机制中发挥了重要作用,病理性瘢痕的组织纤溶系统学说为瘢痕的康复治疗提供了新的理论。结论:瘢痕的纤溶系统理论观点新颖,立论充分,有望为瘢痕的康复治疗提供新的参考。OBJECTIVE: To explore the role of plasminogen activator and plasminogen activator inhibitor in the metabolism of fibrin, and analyze the effect of alternation of extracellular matrix on the pathologic mechanism of scar. DATA SOURCES; A computer-based search was conducted in CNKI full-text database for Chinese articles about the pathologic mechanism of scar published between January 1979 and May 2006 with the key words of "scar, flbdn, plasminogen activator, plasmlnogen activator inhibitor". Meanwhile relevant English articles were searched in Pubmed with the key words of."scaroflbrin,uroklnase-type plasminogen activator, plasrninogen activator inhibitor-1". STUDY SELECTION: Data were checked in the first trial, and literatures about fibrin and scar were included and looked for the full text. Inclusion criteria: Literatures related to the pathological mechanism of scar. Exclusion criteria: Clinical treatment. DATA EXTRACTION: A total of 692 articles related to fibrin and scar were retrieved, including 217 English articles, and 16 enrolled articles in accordance with the criteria were reviewed. DATA SYNTHESIS: Pathologic scar is closely related with trauma, which may be resulted from the prolonged acute reaction period caused by abnormal deposit of fibrin. There is a fibdnolysis system in the scar tissue, which is mainly constituted of urokinase plasminogen activator (uPA) and plasminogen activator inhibitor-1 (PAI-1). The imbalance of uPA and PAl-1 results in the insufficient degradation of fibrin. The fibrinolysis system may play an important role in pathologic mechanism of scar and provide a new theory for the rehabilitation therapy ofpathologic scar. CONCLUSION: The theory of fibrinolysis system is new and persuasible, which may provide new scientific reference for the rahabllitatlon therapy of pathologic scar.
