新生儿缺氧缺血性脑病血清血管内皮生长因子、肌酸磷酸激酶脑型同工酶的表达与临床意义

摘　　要：目的探讨新生儿缺氧缺血性脑病（HIE）时血清血管内皮生长因子（VEGF）、肌酸磷酸激酶脑型同工酶（CK-BB）的动态变化及与病情程度、预后的相关性。方法57例HIE的足月新生儿为HIE组，20例正常健康足月新生儿为对照组。分别在出生0h及生后24h、72h、7d采集空腹静脉血2ml，采用酶联免疫法检测VEGF，电泳法检测CK．BB。结果HIE组血清VEGF浓度在0h表迭为最高峰，与对照组比较差异有显著性（P〈0．01），以后下降；72h、7d时差异无显著性（P〉0．05）；含量随病情加重而升高。CK-BB表达在0h已有增高，于72h达到最高峰，差异有显著性（P〈0．01）。持续血清高水平VEGF、CK-BB提示预后不良。结论VEGF、CK-BB在HIE时表达增加。VEGF的高表达对HIE的早期诊断、病情程度判断及预后评估具有重要参考价值。Objective To explore the dynamic changes of vascular endothelial growth factor（VEGF） and the brain type isoenzyme of creatine phosphokinase （CK-BB） in hypoxic-ischemiceneephalopathy （ HIE ） and evaluate the patients＇ condition and prognosis. Methods 57 new-born neonates diagnosed with HIE （ HIE group ） were selected as the experimental group and 20 full-term new-born healthy neonates were served as the controls. 2 ml blood samples of all the new-born neonates were collected at the born time,24 h,72 h and 7 d after birth respectively. VEGF was examined with enzyme linked immunosorbent assay（ELISA） and CK-BB was determined with enzyme immunoassay. Results The concentration of VEGF in the serum of the patients in HIE group was at peak at 0 h （P 〈0.01 ） and then descended. After born for 72h and 7d, there was no statistical difference between the groups （ P 〉0.05 ）. The concentration of VEGF went higher with the patients＇ condition getting worse. The concentration of CK-BB in the serum of the patients with HIE was at a higher level at 0h,and peaked at 72h（ P 〈 0.01 ）. Continuous high level of VEGF and CK-BB indicated poor prognosis. Conduslon VEGF and CK-BB are increased in the ser- um of the new-born neonates with HIE ,which indicate that the increase of VEGF and CK-BB has great value for the early diagnosis ,judging the degree of the brain injury and predict the prognosis of the patients with HIE.
