B细胞特异性激活蛋白Pax-5在淋巴瘤组织中的表达及其意义

摘　　要：目的探讨B细胞特异性激活蛋白Pax-5在淋巴瘤组织中的表达及其意义。方法运用免疫组化方法对前驱B淋巴母细胞淋巴瘤(B-LBL)、弥漫性大B细胞淋巴瘤(DLBCL)、淋巴浆细胞淋巴瘤(LPL)、浆细胞瘤(PCL)、套细胞淋巴瘤(MCL)、滤泡淋巴瘤(FL)、前驱T淋巴母细胞淋巴瘤(T-LBL)、NK/T细胞淋巴瘤(NK/T)、周围T细胞淋巴瘤(PTL)、间变性大细胞淋巴瘤(ALCL)和经典型霍奇金淋巴瘤(CHL)进行Pax-5和CD20检测,并同时对CHL进行CDl5和CD30检测。结果8例B-LBL中,Pax-5全部(+),6例CD20(-);115例DLBCL中,4例Pax-5(-),3例CD20(-);4例LPL、21例MCL和11例FL中,Pax-5和CD20全部(+);10例PCL、11例T-LBL、25例NK/T、37例PTL、9例ALCL中,Pax-5和CD20均(-)。在非霍奇金淋巴瘤中Pax-5和CD20的表达率差异不显著(P>0·05)。41例CHL中37例RS细胞Pax-5(+),12例CD20(+),Pax-5与CD20的表达率差异显著(P<0·01)。结论Pax-5是一个特异性和敏感性都很高的B细胞特异性标记,具有辅助诊断B-ALL/B-LBL及鉴别诊断CHL和ALCL的重要价值。Objective To investigate the expression and its significance of B cell-specific activator protein （Pax-5） in lymphomas. Methods Pax-5 and CD20 were immunohistochemically detected in 8 eases of precursor B lymphoblastic lymphoma （B-LBL）, 115 cases of diffuse large B call lymphoma （DLBCL）, 4 cases of lymphoplasmaeyfie lymphoma （LPL）, 10 cases of plasmacytoma （PCL）, 21 cases of mantle cell lymphoma （MCL）, 11 cases of follicular lymphoma （FL）, 11 cases of precursor T lymphoblaslie lymphoma （T-LBL）, 25 eases of NK/T-cell lymphoma （NK/T）, 37 eases of peripheral T-eel lymphoma （PIL）, 9 cases of anaplastie large cell lymphoma （ALCL） and 41 cases of classical Hodgkin＇s lymphoma （CHL）, while DC15 and CD30 were also detected in CHL cases. Results In the 8 cases of B-LBL, Pax-5 were all positive in tumor cells among which6 cases CD20 were negative. In the 115 cases of DLBCL, Pax-5 and CD20 were negative in 4 cases and 3 cases, respectively. 4 cases of LPL, 21 cases of MCL,11 cases of FL all expressed Pax-5 and CD20; Pax-5 and CD20 were negative in 10 cases of PCL, 11 cases of T-LBL, 25 cases of NK/T, 37 eases of PIL, 9 cases of ALCL; there was no significant difference between Pax-5 and CD20 expression in the non-Hodgldn lymphom,as （P〉0.05）. In 41 eases of CHL, RS cells expressed Pax-5 and CD20 in 37 cases and 12 cases, respectively, and there were significant difference between them （ P 〈 0.001）. Pax-5 is highly specific and sensitive as a B-cell marker. It may play an important role in the diagnosis of B-ALL/B-LBL and differential diagnosis of CHL and ALCL.
