C-erbB-2在膀胱癌中的表达与CPA血流的关系

摘　　要：目的探讨C-erbB-2癌基因在膀胱癌中的表达与彩色血管能量成像(CPA)血流的关系。方法术前应用CPA检测50例膀胱癌患者及20例膀胱非肿瘤性增生患者的血流信号及其阻力指数RI,术后用免疫组化法术后测该组织中C-erbB-2的表达。结果C-erbB-2在膀胱癌中的阳性表达率为60%。显著高于非肿瘤性增生组(30%,P<0.05),C-erbB-2在II-III级膀胱癌阳性表达率(71%)显著高于0-I级(30%,P<0.05),C-erbB-2在T2-T4期的膀胱癌的阳性表达率(70%),显著高于Tis-T1(30%,P<0.05),膀胱癌的C-erbB-2表达强度与CPA血流分级呈正相关(r=0.677,P<0.05),膀胱癌的RI值(0.50±0.43)显著低于非肿瘤增生性组RI值(0.69±0.41,P<0.05),其表达强度与RI呈负相关(r=-0.537,P<0.05)。结论膀胱癌在C-erbB-2中的表达为临床应用CPA提供了可靠的病理学基础,而CPA则为临床诊断膀胱癌提供了敏感的血流信息。Objective To evaluate the relationship between C-erbB-2 expressions in bladder carcinoma and color power Doppler angio imaging （CPA） blood flow. Methods Fifty patients with bladder carcinoma and 20 cases with bladder hyperplasia underwent CPA preoperatively whereas C-erbB-2 was stained immunohistochemistry after operation. Results C-erbB-2 positive expression rate （60%） in bladder carcinoma was higher than that （30%） in hyperplasia group （P〈0.05）, the C-erbB-2 positive expression （71% ） in grade Ⅱ-Ⅲ was significant higher than that （30%） in grade 0-Ⅰ （P〈0.05）. Similarly, the expression in the stage of T2-T4 （80%） was significant higher than that in the stage of Tis-T1 （30%, P〈0.05） . The C-erbB-2 intensity of bladder carcinoma showed a positive correlation to the CPA degrees （r=0. 677,P〈0.05）. The RI value of malignant group was slightly lower than that （0. 50±0.43 vs 0.69±0. 41） of the hyperplasia group （P〈0. 05） . The intensity of C-erbB-2 showed a negative correlation to RI values （r=-0. 537, P〈0. 05） . Conclusions The expressions of C-erbB-2 for bladder carcinoma may support CPA pathologically and the latter may provide in turn the sensitive blood flow information for the diagnosis of bladder carcinoma in clinical.
