摘 要:目的 研究白介素-1受体拮抗剂(IL—1ra)治疗角膜碱烧伤的免疫学机制。方法Wistar大鼠角膜碱烧伤模型,每组30只。Ⅰ组为阴性对照组,Ⅱ-Ⅳ组为IL-1ra结膜下注射治疗组(1000、500、200μg/ml,2次/d)。在烧伤后不同时间段(3、7、21d)收集角膜,行苏木精一伊红染色,CD3、CD4、CD8阳性细胞计数。结果IL—1ra治疗组角膜水肿程度轻,CD3+、CD4+、CD8+细胞计数在各个时间点均少于对照组(P〈0.05)。7d、21d时1000μg/ml和500μg/ml组之间细胞计数无统计学差异(P〉0.05),但均少于200μg/ml组(P〈0.05)。结论 IL-1ra可剂量依赖性地抑制淋巴细胞在角膜的聚集和活化.下调角膜碱烧伤后免疫应激水平。Objective Immunoinflammatory reaction is a pathologic basis of alkali-burn-mediated cornea damage. Proinflammatory cytokine,or interleukin-1 (IL-1) ,plays an important role in promoting the development of immunoinflammatory lesion following corneal alkali burn. The aim of this paper was to investigate the curative effect and immunology mechanism of interleukin-1 recipient antagonist (IL-lra) treating alkali-burned corneas. Methods Models of corneas alkali-burn were established in Wistar rats by covering the filter paper with 1 nmol/L NaOH for 3 seconds. The animal models were divided into 4 groups randomly and 30 for each. Normal saline was subconjunctivally injected after burning of cornea in control group. IL-1 ra of different concentrations (1 000,500,200 μg/ml) were injected at the same way in various experimental groups respectively for twice per day. Corneal opacity was assessed using a standard grading scheme in 3,7,14 and 21 days. Ten rats corneas were collected at day 3,7 and 21 for histological observation and analysis by HE staining and immunohistochemistry. CD3 + , CD4 + and CD8 + lymphocytes on burned corneas were counted. ResuIts Inflammatory reaction in the eyes of control group was seen obviously,showing massive inflammatory cells infiltration and new blood vessels formation. The CD positive cells were observed in alkali-burned corneas in every group,exhibiting a brown staining in cytoplasm. In IL-1ra treatment groups,inflammation reaction of corneal stroma was inhibited in some extent and the counts of CD3 + , CD4 + and CD8 + cells at different time points were reduced in comparison with control group ( P 〈 0. 05 ). No statistically significant difference was found in IL-1 ra among treated groups at day 3 (P 〉0.05). The counts of CD3 + ,CD4 + and CD8 + cells in 1 000 or 500 μg/ml IL-lra group were decreased compared with 200 μg/ml group at day 7 and 21 (P 〈 0.05 ) , and no distinct difference was seen between 1 000 μg/ml group and 500 μg/ml of
