氩激光联合口服卵磷脂络合碘治疗糖尿病视网膜病变

摘　　要：目的 观察卵磷脂络合碘联合氩激光治疗糖尿病视网膜病变（DR）的疗效。方法 将68例136眼确诊为Ⅲ-Ⅳ期DR的患者随机分为卵磷脂络合碘联合激光光凝组和单纯激光光凝组。高血糖治疗均采用口服降糖药，不用胰岛素。激光治疗后，卵磷脂络合碘联合激光光凝组给予口服卵磷脂络合碘片3个月；单纯激光光凝组只给氩激光治疗。定期复查患者的视力和视网膜水肿、渗出及出血的情况。结果 治疗前两组间视力无显著统计学差异（P〉0．05），治疗后卵磷脂络合碘联合激光光凝组视力优于单纯激光光凝组（P〈0．05）。治疗有效者卵磷脂络合碘联合激光光凝组63眼（92．6％），单纯激光光凝组50眼（73．5％），经统计学分析，前者疗效优于后者（P〈0．05）。卵磷脂络合碘联合激光光凝组用药后未见甲状腺功能异常改变，T3、T4均值与治疗前比较无显著统计学差异（P〉0．05）。结论 卵磷脂络合碘联合氩激光治疗DR安全可靠。Objective Diabetic retinopathy is an ocular microvascular disorder and one of major causes leading to blindness. Argon laser photocoagulation is an effective management for diabetic retinopathy. Jolecithin is proved to reduce the blooding,edema and exudation of retina. We evaluated the efficacy and safety of jolecithin as a adjunct therapy of argon laser for diabetic retinopathy. Methods Sixty-eight consecutive cases （ 136 eyes） with stage UI-IV of diabetic retinopathy were enrolled. The patients were divided into two groups randomly. Jolecithin were orally administrated three times per day for three months after argon laser coagulation in the experimentation group, and argon laser alone was used as control group. The changes of vision acuity, fluorescence fundus angiography （FFA）, photograph of fundus, the function of liver, kidney and thyroid of these patients were observed before and after the utilization of Jolecithin. Results The vision acuity of 45 eyes （ 66.2% ） in experimental group and 37 eyes（54.4% ） in control group was significantly improved after treatment with Jolecithin, showing a statistical difference between two groups（ P 〈 0. 05 ）. The disappearance of retinal blooding, exudation and edema was in 63 eyes （92. 6% ） in Jolecithin-treated group and 50 （73.5%）in control group with a statistically significant difference between them（ P 〈0, 05）, No diversity of the thyroid function was found in the patients of two groups. Conclusion Jolecithin is an effective treatment drug for diabetic retinopathy.
