低位Duck A期直肠癌手术方式与局部复发及预后的关系分析

摘　　要：目的探讨低位Duck A期直肠癌行保肛手术和腹会阴联合根治术(Miles手术)对局部复发率和生存率的影响。方法回顾性分析72例低位Duck A期直肠癌的临床资料。结果30例行Miles手术,42例行保肛手术;局部复发率为6.94%,其中腹会阴联合根治术组为6.67%,保肛术组为7.14%;3年总生存率为95.8%,其中Miles手术组为96.7%,保肛手术组为95.2%。两种手术方式的局部复发率和3年总生存率差异均无统计学意义(均P>0.05)。结论低位Duck A期直肠癌保肛手术和Miles手术对局部复发率和3年生存率无明显影响。Objective To compare the local relapse and the survival rate within 3 years between patients with lower rectal cancer of Duck stage A after sphincter-preserving surgery and Miles surgery. Methods The clinicopathologic factors of 72 patients with lower rectal carcinoma of Duck stage A were retrospectively analyzed. Results Including operineal resection for rectal cancer in 30 cases and sphincter-preserving surgery in 42 cases. The local relapse rate was 6.94 %, and it was 6.67 % and 7.14 % in the patients received operineal resection for rectal cancer and sphincter-preserving surgery respectively. The total survival rate within 3 years was 95.8 %, and it was 96.7 % and 95.2 % in the patients received Miles surgery and sphincter-preserving surgery respectively. The results of chisquare test showed that the local relapse rate and the survival rate within 3 years were not significantly different between patients with lower rectal cancer of Duck stage A after sphincter-preserving surgery and Miles surgery（ P 〉 0.05）. Conclusion The local relapse rate and the survival rate within 3 years has no significant difference between patients with lower rectal cancer of Duck stage A after sphincter-preserving surgery and Miles surgery.
