摘 要:目的探讨不同类型的牙齿重度磨耗患者咀嚼肌肌电活动的特点。方法选择31例牙齿重度磨耗的患者,根据后牙咬合支持状态分成后牙支持完全组(Ⅰ组)和后牙支持不全组(Ⅱ组),以16例正常人为对照,对比分析下颌息止位、正中位大力紧咬以及咀嚼运动时嚼肌、颞肌前束和颞肌后束的表面肌电图特点。结果①Ⅰ组患者在息止位、大力紧咬及咀嚼运动时,嚼肌、颞肌前后束的肌电值与正常组均无明显差别。②Ⅱ组患者在息止位时颞肌前后束肌电值均高于其它组,其中颞肌前束的肌电值Ⅰ组与Ⅱ组之间的差别有显著性(P<0.05);大力紧咬时Ⅱ组嚼肌、颞肌前、后束肌电值均显著低于正常组(分别P<0.01,P<0.01,P<0.05);咀嚼运动时Ⅱ组患者嚼肌、颞肌前、后束的肌电值均明显低于Ⅰ组和正常组。其中嚼肌和颞肌后束Ⅰ组与Ⅱ组之间、颞肌前束Ⅱ组与正常组之间的差别均有显著性(P<0.05)。结论后牙支持是否完整是影响重度磨耗患者咀嚼肌功能的主要因素,后牙支持不全者肌紧张度增加,功能下降。Objective To investigate the EMG activity character of masticatory muscles in patients with different types of tooth wear. Methods Thirty - one patients with severe tooth wear were selected and divided into two groups: group Ⅰ with complete posterior tooth support and group Ⅱ without complete posterior support. The surface electromyography(EMG) of masseter(MM), anterior (TA)and posterior temporalis(TP) were compared with that of 16 normal individuals. Results ①Whenever at rest position or during maximum clenching and chewing, the EMG activities of MM, TA and TP of group Ⅰ had no differences with that of normal group. ②At rest position, the EMG of TA and TP of group Ⅱ were obviously higher than that of normal group and group Ⅰ, in which the difference of EMG of TA between group Ⅰ and Ⅱ was significant ( P 〈 0.05) ; During maximum clenching, the EMG of MM, TA and TP of group Ⅱ were significantly lower than that of normal group (respectively P 〈 0.01, P 〈 0.01, P 〈0.05). The EMG of MM of group Ⅱ was also significantly lower than that of group Ⅰ( P 〈0.05) ; During chewing, the EMG of MM, TA and TP of group Ⅱ were lower than that of other two groups. In which the EMG differences of MM and TP between group Ⅱ and Ⅰ were significant ( P 〈 0.05). Between group Ⅱ and normal, the difference of the EMG of TA was also significantly ( P 〈0.05).Conclusion In patients with severe tooth wear, absence of complete posterior tooth support was the main factor that influenced the functions of patients' masticatory muscles. Those with incomplete posterior support had higher muscle tension and lower muscle functions.
