维甲酸与中性蛋白酶联合防治外伤性增生性玻璃体视网膜病变的研究

摘　　要：目的研究维甲酸与D ispase(中性蛋白酶)分别单独和联合应用对兔外伤性增生性玻璃体视网膜病变(prolifer-ative vitroretinopthay,PVR)的防治作用。方法20只健康日本大耳白兔,随机分成4组,即对照组、维甲酸组、D ispase组及联合用药组。制备外伤性PVR模型后,各治疗组玻璃体腔注入相应药物,对照组注入生理盐水,观察和比较药物防治外伤性PVR的疗效。结果用药组PVR平均增生级别均低于对照组,与对照组比较差异有统计学意义(P<0.05),单独用药组PVR平均增生级别高于联合用药组,差异有统计学意义(P<0.05)。其中维甲酸组及联合用药组视网膜前的增生细胞数及新生血管数少于对照组及D ispase组,差异有统计学意义(P<0.05)。结论作用机制不同的药物维甲酸和D ispase可防治外伤性PVR的发生、发展,两者联合应用有一定的协同作用。Objective To investigate the preventing effect of retinoic acid and Dispase on an experimental model of traumatic proliferative vitreoretinopathy （ PVR ）. Methods Twenty white Japanese big ear healthy rabbits were randomly divided into four groups as follows：the control group,the retinoic acid group,the Dispase group and the retinoic acid ＋ Dispase group. After making experimental model of PVR, the treated group were intravitreal injected with respective drug, the control groups＇ eyes were injected with saline. The efficacy of the inhibition of PVR was evaluated according to grade of PVR and light microscopy. Results Significant statistical differences were found between the control group and other three treated groups （P 〈 0.05 ）. The proliferative grades in the retinoic acid group and the Dispase group were significantly higher than the those in retinoic acid ＋ Dispase group （P〈 0.05 ）. The number of proliferated cell and neovascular vessel in the retinoic acid group and the retinoic acid ＋ Dispase group were significantly fewer than those in the control group and the Dispase group （ P 〈 0. 05）. Conclusions The development of PVR in this model was inhibited effectively by retinoic acid and Dispase. The combination use of these two drugs has a certain cooperative action in the treatment of PVR.
