摘  要:目的探讨因子与校准品校准在血清酶检测中的差异。方法应用速率法,采用因子校准与校准品校准2种校准方式,对肌酸激酶(CK)、乳酸脱氢酶(LDH)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、γ-谷氨酰转移酶(GGT)等5种酶进行检测,室内定值质控血清批内各测20次,随机标本50份分别进行检测,在1 h内完成,对2种校准结果进行比较。结果CK、LDH、ALT、AST、GGT表现为因子校准结果明显低于校准品校准结果。室内定值质控血清因子校准均值分别为CK 191.4 U/L、LDH 178.5 U/L、ALT 29.0 U/L、AST 32.8 U/L、GGT50.6 U/L,校准品校准均值分别为CK 212.2 U/L、LDH 183.4 U/L、ALT 32.0 U/L、AST 35.4 U/L、GGT 55.9 U/L;随机标本检测因子校准均值分别为CK 110.6 U/L、LDH 208.1 U/L、ALT 80.3 U/L、AST 99.8 U/L、GGT84.1 U/L,校准品校准均值分别为CK 133.2 U/L、LDH 215.0 U/L、ALT 95.8 U/L、AST 115.6 U/L、GGT 94.8 U/L。两法比较差异有统计学意义(P均<0.001)。结论通过对定值质控血清及随机标本所测酶结果的分析,认为酶活性检测因子校准有较大的系统误差,校准品校准能更好地反映结果的准确性。Objective To study the difference between the calibration using enzyme activity factor and calibrator. Methods The activity of creatine kinase( CK), lactic dehydrogenase( LDH), alanine aminotransferase( ALT), aspartate aminotransferase( AST), γ-glutamyhransferase(GGT) were determined by rate method using two calibration methods of factor and calibrator. Internal quality control pooled serum was determined for 20 runs and 50 random samples were determined. The assay completed in one hour, then the results with the two calibrator were compared. Results The results of factor calibration of CK, LDH, ALT, AST, GGT were significantly lower than that with calibrator. The resuits of factor calibration of internal quality control pooled serum factor were CK 191.4 U/L, LDH 178.5 U/L, ALT 29.0 U/L, AST 32.8 U/L, GGT 50.6 U/L; the results with calibrator were CK 212.2 U/L, LDH 183.4 U/L, ALT 32.0 U/L, AST 35.4 U/L, GGT 55.9 U/L. The results of factor calibration of random sample factor were CK 110.6 U/L, LDH 208.1 U/L, ALT 80.3 U/L, AST 99.8 U/L, GGT 84.1 U/L; the results with calibrator were CK 133.2 U/L, LDH 215.0 U/L, ALT 95.8 U/L, AST 115.6 U/L, GGT 94.8 U/L. There was significant difference between the two methods (P 〈 0.001 ). Conclusions Enzyme activity factor calibration has greater system error. Calibrator calibration reflects better the accuracy of the result.
