肝动脉化疗栓塞联合三维适形放疗治疗单发肝癌31例临床疗效观察

摘　　要：目的观察肝动脉化疗栓塞（TACE）联合三维适形放疗（3DCRT）治疗单发肝癌的效果。方法选择单发肝癌患者31例，先行1次TACE，2～3周后行1个疗程3DCRT，然后再行TACE2—3次。另取46例肝癌患者单纯行TACE作为对照。随访3个月、0．5年、1年评价疗效。结果单纯TACE组完全缓解（CR）9例，部分缓解（PR）11例，稳定（SD）26例，有效率43．4％，生存期6～27个月。0．5年生存率69．5％，1年生存率36．9％。TACE＋3DCRT组CR10例，PR16例，NC5例。有效率（CR＋PR）83．8％，生存期8～31个月，0．5年生存率达到80．6％，1年生存率为64．5％。两组有效率和生存率比较差异有统计学意义（P值均〈0．05）。结论TACE联合3DCRT治疗单发肝癌，克服了单独应用这两种治疗方法的不足。比单用TACE有效率和1年生存率明显提高。Objective To observe the efficacy of TACE combining 3DCRT in the treatment of singular hepatocellular. Method A total of 31 patients with singular hepatocellular carcinoma firstly underwent a TACE procedure. At 2 weeks to 3 weeks after TACE they underwent a 3DCRT, then they underwent 2 or 3 TACE procedures again. They were followed up 3 months, 6months, and 12 months to evaluate the efficacy. Results Of 31 patients, 10 completely remitted（CR）, 16 partially remitted（PR）and 5 had no change（NC）. The effective rate （CR＋PR） was 83.8 %, and the survival range was 8 months to 31 months. The 0.5 year survival rate is 80.6 %, and the 1 year survival rate is 64.5 %. Conclusions With the advisable combination of TACE with 3DCRT to cure singular hepatocellular carcinoma, we can conquer the defects of using one of them apart, and comparing with TACE （the effection rate 43.4 %, the 1 year survival rate 36.9 %）, the effective rate and 1 year survival rate are improved obviously.
