摘  要:目的探讨视网膜母细胞瘤(retinoblastoma,Rb)基因和S期激酶相关蛋白2(S-phasekinaseasso-ciatedprotein2,SKp2)在宫颈鳞癌及子宫颈上皮内瘤变(cervicalintraepithelial,CIN)中的表达情况及其意义。方法应用免疫组化技术(S-P法),检测Rb基因产物和SKp2在各级别CIN和宫颈鳞癌中的表达状况,并以正常子宫颈鳞状上皮(简称正常组)做对照。结果正常宫颈鳞状上皮20例、CINⅠ20例、CINⅡ20例、CINⅢ37例及宫颈鳞癌40例,Rb基因蛋白和SKp2表达总阳性率分别为100.0%,55.0%,20.0%,24.3%、23.5%和45.0%,80.0%,70.0%,94.6%,92.5%。Rb和SKp2的表达组间统计均存在显著性差异(P<0.005)。另外Rb在正常组与CINⅠ、CINⅡ、CINⅢ、宫颈鳞癌组表达存在显著性差异(P<0.005);在CINⅠ组与CINⅢ、宫颈鳞癌组间表达存在显著性差异(P<0.005)。SKp2在正常组与CINⅢ、宫颈鳞癌组间表达存在显著性差异(P<0.005);在CINⅠ组与CINⅢ、宫颈鳞癌组间表达存在显著性差异(P<0.005);在CINⅡ组与CINⅢ、宫颈鳞癌组间表达存在显著性差异(P<0.005)。结论Rb基因蛋白在正常组100%表达强阳性,而随着发生CIN从轻到重至宫颈鳞癌,表达逐渐减弱,甚至消失。证明Rb基因可能主要通过功能性失活或缺失参与宫颈癌的发生,在癌变过程中起重要作用。相反,SKp2在正常组表达阳性率较低,而随着发生CIN从轻到重至宫颈鳞癌,表达呈逐渐增强趋势,表明SKp2在子宫颈癌癌变过程中起促进作用。尤其临床病理实践中检测到Rb表达减弱或消失,而SKp2表达增强,有助于早期子宫颈癌的判断,在妇产科临床病理上,2个指标对于辅助诊断子宫颈癌和CIN非常有价值。Objective: To investigate the expression of retinoblastoma (Rb) gene and S -phase kinase associated protein 2 (SKp2) in cervical invasive squamous cell carcinoma (CISCC) and cervical intraepithelial neoplasia ( CIN), explore the significance of its expression in the carcinogenesis of CISCC. Methods : Rb and SKp2 were studied with immunohistochemical staining ( S - P) in 77 cases of CIN and 40 cases of CISCC, 20 cases of normal cervical epithelia tissue. Results: The positive rates of Rb decreased from normal cervical epithelia, CIN to CISCC, the rates were 100.0%, 55.0% , 20.0% , 24.3% and 23.5% respectively. There were significant difference among normal cervical epithelia with CIN and CISCC groups ( P 〈 0. 005 ) ; the positive rates of Rb in CISCC and CIN Ⅲ were significantly higherr than CIN Ⅰ ( P 〈 0.005 ). The positive rates of SKp2 increased from normal cervical epithelia, CIN Ⅰ , CIN Ⅱ , CIN Ⅲ to CISCC groups, their positive rates were 45.0%, 80.0%, 70.0%, 94.6%, 92.5% respectively ; The expression of SKp2 in CISCC and CIN Ⅲ significantly higher than normal cervical epithelia ( P 〈 0. 005 ) ;CIN Ⅲ and CISCC significantly highe than CIN Ⅰ (P 〈0.005 ) ;CIN Ⅲ and CISCC significantly highe than CIN Ⅱ ( P 〈 0. 005 ). Conclusion : Rbgene expressed in all 20 cases of normal cervical epithelia. The Rb protein expression decreases or no expression from normal cervical epithelia, CIN to CISCC. The functional deactivation of Rb involved in the carcinogenesis of CISCC and takes an important role. The weak expression of SKp2 was found in norreal cervical epithelia, the positive rates of SKp2 become higher and higher from CIN to CISCC, the over - expression of SKp2 takes an important role in the carcinogenesis of CISCC. In gynecological pathology, to detect the weak expression of Rb and the over - expression of SKp2 is significant for the diagnosis of CIN and CISCC.
