青海西宁海东地区老年人消化性溃疡1184例内镜下特点

摘　　要：目的:了解青海西宁及海东地区老年人消化性溃疡(PU)的发病情况及内镜下特点。方法:分析西宁及海东地区5年间胃镜检查的临床资料。结果:在20 700例胃镜检查中,检出PU5 781例,检出率为29.73%,受检老年人4 079例,检出PU 1 184例,检出率29.03%,其中,胃溃疡(GU)736例,十二指肠溃疡(DU)400例,复合性溃疡(CU)48例。结论:西宁及海东地区老年人PU检出率比非老年人均较高,但两者间无统计学差别;老年人GU检出率高于DU,老年人GU检出率高于非老年人,老年人DU、CU检出率低于非老年人;老年人三种类型溃疡中GU和DU在男性检出率均显著高于女性;季节以春秋季高于冬夏季,表明PU好发于气温骤变的季节;DU中,以球部前壁为明显多见,其次为球部大弯一侧。GU中,以胃角溃疡最多,其次为胃体小弯侧和胃窦,这与国内文献报告一致。溃疡大小以直径≤10mm为主,77.01%(DU),48.37%(GU),数目以单发为主,DU为83.26%,GU 80.16%。Abstract Objective： To understand the characteristics under endoscopy of elderly patients with peptic ulcer （PU） living in Xining and Haidong areas of Qinghai province. Methods： The clinical data of endoscopy in patients with PU came from Xining and Haidong areas between Jan. 1998 and Dec. 2002 in our hospital were analyzed retrospectively. Results. 5 781 patients with PU were found and were demonstrated by pathology in 20 700 general endoscope examinations. The general detected rate of PU was 29.73%. 4 079 of 5 781 cases were older patients and 1184（ ≥60 years old） of 4 079 cases were consisted of 736 gastric ul- cer （GU）, 400 duodenal ulcer （DU） and 48 complex ulcer （CU）. The detected rate of general PU, GU, DU and CU were 29. 03%, 18. 04%, 9. 81% and 1.18%, respectively, in elderly patients. Conclusions： The general detected rate of PU was higher in all cases in Xi＇ning and Haidong areas compared with that reported from other areas, but no significant difference between the elderly and others （〈60 years old）. The detected rate of GU was higher in the elderly than that in others; contrast in DU or CU. In elderly cases, epidemic attacks of PU occur mostly in spring and autumn,. The detected rate of GU or DU was significantly higher in men than in women. 77. 01% of DU and 48. 37% of GU were equal to or smaller than 10 mm in diameter. The ulcer was single in cases of 80. 16＆ of GU and 83. 26% of DU.
