光学性角膜移植术联合白内障超声乳化人工晶状体植入术临床观察

摘　　要：目的探讨光学性角膜移植术联合白内障超声乳化人工晶状体植入术的安全性及效果。方法通过对角膜瘢痕伴白内障患者14例（14只眼）施行角膜穿透移植联合小切口白内障超声乳化摘出及人工晶状体植入术，在术后不同时期对角膜植片、视力变化和屈光改变、并发症出现与处理进行跟踪观察。结果14只眼术前裸眼视力0．02～0．08。术后第7天视力0．2者2只眼，大于0．2者9只眼，0．5者3只眼。术后6个月，最佳矫正视力0．25者2只眼，其余均在0．3以上，有8只眼在0．5以上。结论角膜移植术联合白内障超声乳化人工晶状体植人术只要选择合适的病例，视功能良好，角膜以中央混浊为主的瘢痕伴早期白内障患者，手术时尽可能散大瞳孔并仔细操作，角膜移植手术联合白内障超声乳化术仍是安全且有效可行的。OBJECTIVE To explore feasibility and efficacy of optic keratoplasty joint micro - incision phacoemulsification with implantation of fold intraocular lens.METHODS Optic keratoplasty joint micro - incision phaeoemulsification with implantation of fold intraocular lens was performed on 14 cases （14 eyes） with corneal scar eyes complicated by cataract. Observations began tracking postoperatively in different time on corneal implant, vision changes, refraction changes, occurrence and treatment of complications. RESULTS Preoperative naked vision of 14 eyes was between 0.02 ～ 0.08, all below 0.1. Seven days after surgery naked vision was 0.2 of 2 eyes; 0.2 ～ 0.5, 9 eyes; 0.5, 3 eyes. After 6 months, corrected vision was 0.25 of 2 eyes; the remaining was all above 0.3, including 8 eyes above 0.5. CONCLUSIONS It is safe, effective and feasible to perform optic keratoplasty joint micro - incision phacoemulsification with implantation of fold intraocular lens as long as the surgical case is appropriate, whose visual function is well and turbidity mainly in the type of central cornea scar complicated by early cataract, and expanding pupil as large as possible in surgery.
