肝癌及肝血管瘤三维彩色能量成像特征的初步研究

摘　　要：目的探讨肝癌和肝血管瘤的三维彩色能量成像特点,寻找声像图上诊断和鉴别诊断肝癌及肝血管瘤的依据。方法对30个肝癌结节和24个肝血管瘤结节进行三维血管能量成像,使用仪器为HDI5000 Sono CT彩色超声诊断仪。结果肝癌组和肝血管瘤组的三维血管能量成像各有特征。肝癌和肝血管瘤均可分为多血管型和少血管型。肝癌分为4种类型:Ⅰ型树枝状,Ⅱ型网状,Ⅲ型放射状,Ⅳ型为少血管型。多血管型肝癌血管多,且粗大紊乱,分支多,终末细小血管丰富。结论三维血管能量成像可以作为肝癌及肝血管瘤鉴别诊断的依据。Objective To research and evaluate the characteristics of the three-dimensional color power angiography（3D-CPA） of hepatic carcinoma and hemangioma for distinguishing them. Methods HDI 5000 Sono CT system and C5 - 2 superwide frequency transducer performed 3D-CPA examination in 30 hepatic carcinomas and 24 hemangiomas. Re.suits 3D-CPA of hepatic carcinomas and hemangiomas have their characteristics respectively. Both of hepatic carcinoma and the hepatic hemangioma can be divided into more blood supplide type and fewer blood supplied type. Hepatic carcinoma can be divided into 4 types. Ⅱtype is tree branch-like, Ⅱtype is ret-like,Ⅲtype is radiation-like, Ⅳ type is fewer blood supplied type. The blood vessels of more blood supplied hepatic carcinoma are multiple,wide and disorderly. Their branch is many,and their terminal blood vessel is plentiful. Conclusions The 3 D-CPA of hepatic carcinoma and the hepatic hemangioma have characteristics respectively,and can be a objective and reliable evidence to distinguish them.
