调对TMD慢性疼痛治疗效果的临床评价初探

摘　　要：目的:探讨调治疗颞下颌关节紊乱病(TMD)慢性疼痛的疗效。方法:TMD伴慢性口颌面痛患者21例,盲法随机分为治疗组(10例)和对照组(11例),治疗组予以本课题组所报道的方法进行调治疗,以建立平稳咬合接触为目标,对照组作模拟调。检测治疗前、治疗后即刻和治疗1月时疼痛指数(VAS)、开口度及咬合力水平。结果:治疗组VAS降低(p=0.000),开口度增加(p=0.008),咬合力有所增加(p=0.050),与对照组相差显著。对照组治疗前后无明显差异。结论:以建立自然牙列稳定咬合接触关系为目的的调治疗可以明显缓解TMD慢性疼痛。Objective：To investigate the effects of occlusal adjustment on temporomandibular disorders （TMD） patients with chronic Orofacial pain.Method： Twenty-one TMD patients with temporomandibular disorders （TMD）chronic orofacial pain, divided randomly into two group, ten in treatment group treated with occlusal adjustment （OA） that aimed to establish a stable occlusal contact condition, and the other eleven in control group with mock occlusal adjustment （MOA）. Visual analog scale （VAS）, maximum mouth opening and bite force level of the bilateral mandibular first molar teeth were tested or measured before, the moment after occlusal treatment and one month later.Result： In ocelusal adjustment treatment group the VAS decreased significantly （P=0.000）, maximum mouth opening （P=0.008） and the bite force level increased significantly （P=0.050）one month after the treatment, differed significantly from mock occlusal adjustment group. While no difference was found between that of immediately after treatment and that of before treatment.Conclusion： Occlusal ajustment that aimed to establish a stable occlusal contact condition is effective in alleviating the chronic orofacial pain of TMD patients.
