附加异体软骨支架矫正下睑瘢痕性外翻

摘　　要：目的:本研究在传统的下睑外翻矫正术中,采用异体软骨内置于外翻矫正中的下眼睑,观察其对瘢痕性下睑外翻的临床矫治效果。方法:于眼轮匝肌浅面或深面植入半月形异体软骨支架。结果:2000年1月至2003年12月间烧伤后下睑外翻患者18例,术后17例患者下睑外翻得到彻底矫正,另仅1例因植皮后皮片存活欠佳,导致异体软骨外露,取出后重新植皮修复,4月后再度下睑外翻,经上法修复。经过6个月以上的随访,眼裂启闭功能良好,下睑外翻未复发。结论:在传统的瘢痕性睑外翻矫正术中附加异体软骨支架支撑下睑,可以拮抗其表面皮肤组织的挛缩,防止再度出现下睑外翻。Objective In this study, we used the allo-cartilage bracket to repair the lower eyelid cicatricial ectrophia, and investigated its clinical effect. Methods After loosening the scar conglutination in the lower eyelid completely, the lune-shape allo-cartilage bracket were placed above or under the M orbicularis oculi, and were supported by the wall inferior orbitalis. The wounds were covered by local skin flaps or skin graft. Results From Jan 2000 to Dec 2003, 18 patients with lower eyelid ectropion after burn injury were admitted in our hospital. Among them, 17 cases with lower eyelid ectropion were corrected completely. Only in 1 case, the skin graft was unsuccessful and the allo-cartilage exposed. After that, the allo-cartilage was removed and covered the wound by graft. Lower eyelid ectrophia reappeared 4 months later, and corrected by the above method. After more than 6 months follow-up, rima oculi can open and close freely, and. lower eyelid ectrophia did not recur. Conclusion After the lower eyelid was supporter by the allo-cartilage bracket, it could antagonize the skin contracture and prevent the recurrence of the lower eyelid ectrophia, which coulddiminish the chance of injury in eyeball and the pain of repeated operation.
