摘  要:目的探讨准分子激光原位角膜磨镶术(laser in situ keratomileusis,LASIK)对黄斑的影响。方法随机选择接受LASIK治疗的中高度近视患者48眼,术前屈光度数(等效球镜)平均为(-7.43±1.38)D,于术前、术后第3天、第10天、第1个月行海德堡视网膜地形图系统(Heidelberg retinal tomography,HRT)-Ⅱ检查,监测黄斑中心凹、0.5mm区、1mm区视网膜厚度的变化。结果黄斑中心凹厚度术前、术后第3天、第10天、第1个月分别为(0.80±0.24)mm、(0.96±0.27)mm、(0.92±0.26)mm和(0.83±0.23)mm,手术前后相比,术后第3天、第10天较术前增厚,差异有显著性(P<0.05),术后第1个月与术前相比无明显差异(P>0.05),同时术后第10天的增厚与术中切削深度存在显著相关性(r=0.286,P=0.049)。而黄斑0.5mm区和1mm区视网膜厚度手术前后无明显差异(P>0.05)。结论LASIK术对黄斑中心凹存在着暂时性的轻微的影响,1个月后恢复正常,并无远期的不可逆性损伤。Objective To invstigate changes in the macule after LASIK and the factors which affect the outcome. Methods This study was comprised of 48 eyes with maderate and high myopia that had undergone LASIK. Rethnal thickness of the macula was measured with an HRT-Ⅱ at pre-operation, and post-operation 3 days, 10 days and 1 month. The thickness of the fores, a 0.5mm area and a 1 mm area, was impected.Results The mean postoperative thickness of the macular fovea was (0.96 ± 0.27)mm, (0.92±0.26)mm, and (0.83±0.23)mm 3 days, 10 days and 1 month, respeczively, while the baseline was (0.80 ± 0.24)mm. There were significant differences in the retinal thickness of the macular fovea between pre-operation and both post-operation 3 and 10 days (P〈 0.05). The increase in retinal thickness of the macular fovea post-operation 10 days correlated with ablatad depth during LASIK ( r = 0. 286, P = 0. 049). Conclusion The effect of LASIK on the change in macula thickness is mild and temporary. Mild macular foveal edema is common after the operation and the macular fovea recovers by one month after LASIK.
