NSCLC中EMMPRIN表达与淋巴结转移和预后的关系

摘　　要：目的探讨细胞外基质金属蛋白酶诱导因子（EMMPRIN）蛋白的表达与非小细胞肺癌（NSCLC）淋巴结转移和预后的关系。方法用免疫组织化学方法检测77例手术切除的NSCLC组织中EMMPRIN的表达。分析EMMPRIN表达、患者吸烟指教、肿瘤T分期、N分期、组织学类型、组织分化程度、淋巴结转、术后化疗周期教和患者预后之间的关系。结果77例NSCLC患者到随访结束时死亡率72．73％（56／77），术后生存时间1—84个月、中位生存时间为19月、三年生存率为32．47％（25／77）；EMMPRIN的阳性表达率为67．53％（52／77）；EMMPRIN的表达与淋巴结转移呈正相关（P=0．001。r=0．371），与患者手术后生存期呈负相关（P=0．000）。无淋巴细结转移组NSCLC的中位生存时间和三年生存率高于有淋巴结转移组（P=0．001）。在COX多因素分析中，癌肿痛理类型、EMMPRIN蛋白表达和肺癌术后化疗周期数均为影响生存率的重要因素。结论NSCLC的淋巴结转移、病理类型和术后化疗周期与预后有密切关系；EMMPRIN蛋白的表达与NSCLC的淋巴结转移和预后密切相关。Objective To investigate the relationship of expression of extraeellular matrix metalloproteinase inducer （EMMPRIN） with lymphoid metastasis and prognosis in non - small- cell lung eareinoma（NSCLC）. Methods Expression of EMMPRIN in 77 cases of patients with NSCLC was immunohistochemieally detected. The relationship among the expression of EMMPRIN, smoking index number, tumor T stages, N stages, histological type, differentiation, lymphoid metastasis, the cycles of adjuvant chemotherapy and prognosis was analyzed. Results The overall of fatality rate of NSCLC patients was 72,73 % （56/77）at the end of follow - up, the postoperative survival duration was from 1 to 84 monthes, the median survival duration was 19 months, the 3 - years survival rate was 32.47% （25/77） ;The positive expression rate of EMMPRIN in NSCLC was 67.53 % （52/77） ;The expression of EMMPRIN was associated positively with lymphoid metastasis （P = 0.001, r = 0.371 ） and inversely with survival time （ P = 0. 000）. The median survival duration and the 3 - years survival survival in non- lymphoid metastasis group were longer than the lymphoid metastasis group （ P = 0.001 ）. In a COX multivariate analysis, the survival of the patients was obviously influenced by the factors of histological type, expression of EMMPRIN and the cycles of adjuvant chemotherapy, Conclusion The prognosis of NSCLC axe associated with lymphoid metastasis, histological type, he cycles of adjuvant ehemotherapy in NSCLC. The expression of EMMPRIN protein are associated with lymphoid metastasis and prognosis in NSCLC.
