摘  要:目的分析4年间眼部感染的真菌菌属分布及其体外药物敏感性。方法收集2179份眼部标本进行分离培养,对真菌培养阳性率、菌属的种属分布、菌属的季节分布及年龄与性别分布进行回顾性分析;同时分析培养阳性的真菌对常用抗真菌药物的敏感性。结果共培养阳性菌株681株,阳性率为31.25%,其中镰刀菌属394株(57.86%),曲霉菌属116株(17.03%)。培养阳性的标本中,角膜591株(86.78%)、房水29株(4.26%)、结膜22株(3.23%)、玻璃体22株(3.23%)、泪囊1株(0.15%)、其他16株(2.35%)。体外药物敏感性观察发现,镰刀菌中368株对那他敏感(93.40%),曲霉菌中107株对伊曲康唑敏感(92.24%)。结论镰刀菌属及曲霉菌属仍是我国北方主要的眼部致病真菌菌属;镰刀菌对那他霉素最敏感,曲霉菌对伊曲康唑最敏感。Objective Ocular fungal infection is one of the most common infectious eye diseases in China. Severe mycotic ocular diseases still were very difficult to be managed. In this paper, we analyzed the distribution characteristics of ocular fungal isolates and susceptibility of anti-fungal agents. Methods 2 179 specimens obtained from 2001 to 2004 were identified at Beijing Institute of Ophthalmology. Fungal culture-positive rate, susceptibility patterns, the genus distribution of positive cultures were analyzed retrospectively. The gender and age of the patients, the location of ocular involvement, the year and the season of onset were investigated for the fungal culture-positive samples. Results Out of 2 179 specimens,681 specimens were positive outcome with the culture-positive rate 31.25%. Ninety-two (13.51%)strains were collected from January to March, 134 (19.67%) from April to June, 163 (23.94%)from July to September,and 292(42. 88% )from October to December. In 681 positive culture results,591 specimens were from the cornea,29 from the aqueous humor,22 from the conjunctiva,22 from the vitreous body, 1 from the lacrimal sac and 16 from the other location of the eyes. Fusarium sp was the commonest pathogen in 394 ( 57.86 % ) , followed by Aspergirum sp in 116 ( 17. 03 % ). Nineteen ( 2. 80% ) strains were from the patients 〈 14 years old, 244 (35.83%)from 15 -40 age group,351 (51.54%)from 41 -60 age group and 67(9. 83% )from the patients 〉61 years old. 451 positive specimens were isolated from male, and 230 from female. The sensitivity in vitro to natamycin, terbinafine, fluconazole, itraconazole were 608/681 ( 89.28% ) ,46/ 681 ( 6. 75% ) , 102/ 681 ( 14. 985 ) , 260/681 ( 38.18% ) , retrospectively. 368 of Fusarium species were sensitive to natamycin (93.4%) , 107 of Aspergillus species were sensitive to itraconazole (92.2%).
