双吻合器技术在全系膜切除低位直肠癌保肛手术中的应用

摘　　要：目的探讨经过选择的低位直肠癌患者中双吻合器技术在全系膜切除保肛手术中的价值。方法对距肛缘4～7cm的低位直肠癌患者56例行结直肠吻合保肛手术。结果全组无手术死亡,术后发生吻合口漏1例(1．8％),吻合口狭窄7例(12．5％)。术后局部复发3例,其中2例为吻合口复发,1例为盆腔复发,总局部复发率为5．4％。结论只要合理选择手术适应证,全系膜切除结合双吻合器技术在低位直肠癌保肛手术中是安全可靠的。Objective To evaluate the role of total mesorectal excision and double stapling technique in anussaving operation for patients with low rectal cancer. Methods 56 patients with low rectal cancer who had the distance between the anal verge and low margin of the tumor was 4-7cm received anus-saving in colorectal anastomosis after anterior resection. Results There was no operation-related death.There were 1 case （1.8%）of anastomotic leak and 7 cases （12.5 %）of anastomotic stricture.3 cases （5.4%）developed local recurrence including 2 cases at the anastomtic sites and 1 case in plvis.Conclusion The double stapling technique and total mesorectal excision provides a safe alternative for anus-saving operation in some selected patients with rectal cancer.
