远端蒂耳大神经营养血管筋膜皮瓣的测量研究与临床应用

摘　　要：目的探讨远端蒂耳大神经营养血管筋膜皮瓣修复口腔颌面部软组织缺损的临床应用。方法在100名正常成年人头而部相关测量点用游标卡尺进行实体测量。对5例颌面部肿瘤手术后的软组织缺损应用远端蒂耳大神经营养血管筋膜皮瓣修复,评价其临床效果。结果远端蒂耳大神经营养血管筋膜皮瓣修复可用来修复颊部、舌、口底等处的软组织缺损。5例皮瓣全部成活,其中1例远端约1/4的皮肤坏死,随访3个月至1年,外形满意、语言及咀嚼功能良好。结论远端蒂耳大神经营养血管皮瓣可应用于口腔颌面部软组织缺损的修复。Objective To explore clinical application of distally based greater auricular neurocutaneous fascia flap for repairing the soft tissue defects in the oral maxllofacial areas.Methods 100 normal adults were measured by vernier caliper.5 patients with post-surgical soft tissue defects in the oral maxllofacial areas were treated with distally based greater auricular neurocutaneous fascia flap and were evaluated the clinical effects.Results We can repair the soft tissue defects of cheek, lingua, floor of mouth etc with the distally based greater auricular neurocutaneous fascia flap.All the flaps in 5 cases were survived.However,there was partial dermal necrosis in one case.Followed up three months to one year, all the patients were satisfied with the exterior, lingual function and masticatory function.Conclusion The flap has liable blood supply and can be designed to reconstruct the soft tissue defects in head and neck.
