摘  要:目的探讨多层CT肺灌注成像技术在肺癌TNM分期中的临床应用价值。方法收集行MSCT灌注成像的肺癌患者20例,均经手术病理证实,其中鳞癌7例,腺癌11例,小细胞肺癌2例。使用GELight-speedPlus16层CT机行肺CT灌注扫描。应用Perfusion2软件进行灌注成像分析。结果20例肺癌病例的CT灌注图像均比较满意。CT灌注参数对于肺癌的T分期有一定帮助。<3cm的肺癌(T1)血管表面通透性(PS值)明显低于>3cm(T2)的PS值,有统计学意义。胸膜受累的肺癌较胸膜未受累的肺癌血流量(BF)、血容量(BV)及PS值均增加及平均通过时间(MTT)加快;有淋巴结转移的肺癌较无转移的BV增加和MTT加快,而BF和PS相差不大,但是,均缺乏统计学差异。有无远处转移对灌注参数影响不大。结论MSCT灌注成像对于肺癌的T分期有一定帮助。Objective To study the clinical applied value of perfusion imging technique with multi - slice CT in staging of lung cancer. Methods 20 patients with lung cancer confirmed pathologically underwent perfusion imaging with GE Light - speed Plus 16 - slice CT,including 7 cases of squama cancer, 11 cases of adenocarcinoma and 2 cases of small cell lung cancer. The perfusion images were analysed using perfusion - 2 software. Results CT perfusion images in 20 cases of lung cancer were relatively satisfied and its perfusion parameters were helpful for T staging of lung cancer. Permeability surface(PS) value of lung cancer which was less than 3 cm( T1 ) in diameter was obviously less than that the lesions were large more than 3 cm (T2 ) ,there was statistics meaning. Blood flow (BF) ,blood volume(BV) and PS value increased obviously and mean transit time(MTT) value accelerated in lung cancer with invaded pleura than without that. BV value increased and MTT value accelerated in lung cancer with lymph metastasis in comparison with that without lymph metastasis,but BF and PS value had not significant difference between them. With or without distant metastasis does not affect on perfusion parameter. Conclusion Perfusion im- aging with multi - slice CT is helpful to T staging of lung cancer.
