鼻咽癌放疗中甘氨双唑钠增敏与顺铂增敏作用的比较

摘　　要：目的：比较甘氨双唑钠（CMNa）与顺铂（DDP）在鼻咽癌放射治疗中的增敏作用和副作用。方法：首程治疗62例鼻咽癌患者，随机分DDP组及CMNa组，用药方法：CMNa每次按800mg／m^2给药，用100ml生理盐水溶解后，于30min内静脉滴注，60min内进行常规放射治疗。每周一、三、五用药，至常规放射治疗结束为止。DDP每次10mg，用250ml生理盐水溶解后，第1，第2周，1次／d，第5周重复。照射方法：先制作面模固定，在模拟机下设面颈联合野放疗DT36Gy／18次／3．5周，后改设耳前野和鼻前野（或耳后野）放疗总量至DT68—72Gy／34—36次／6．5～7周。结果：CMNa增敏作用与DDP增敏作用P〉0.05无明显差异，但CMNa组消化道反应小，对血常规及肝功能影响不明显P〈0．01。结论：CMNa及DDP均具有放射治疗增敏作用，二者无明显差异，而CMNa组副作用更少。但其长期疗效有待进一步研究。Objective To compare DDP and CMNa in radiosensitivity and side-effect in patients with nasopharyngeal carcinoma. Methods： Sixty-two patients were randomized into two groups, DDP group and CMNa group. CMNa was used venously with 800 mg/m2 in 30 minutes and radiotherapy was performed in 60 minutes in Monday, Wednesday and Friday for every week from the beginning to the end of the radio- therapy. DDP was used venously with lOmg daily from Monday to Friday in first two weeks and in the fourth and fifth week, Facial-neck field was given with 38GY, then facial-jugnlar field and anterior-nasal field were given with 30-34GY. Results： In the two groups the CR ＋ PR for local lesion was 93.3% and 84. 38% respectively ,90.9% and 84.38% respectively for lymph node, and the remote metastasis for the two groups was 16.67% and 18.75%. Conclusion： There was no significant difference in radiosensitivity （P 〉 0.05 ） in two groups but the side-effect in the DDP group was more severe than in CMNa group, And the long term effect needs further study in the future.
