摘 要:目的:分析宫颈癌放射治疗(简称放疗)中与放疗后肿瘤与盆腔非肿瘤软组织的MRI改变。方法:36例经病理证实的宫颈癌患者在放射治疗前、中、和治疗后不同时间阶段行盆腔的MRI成像,并观察肿瘤及盆腔软组织器官在放疗前后的大小或信号改变。结果:1、疗效:放疗中/放疗完成后/完成后2—3个月2/I/0例肿瘤无缩小,32/16/12例肿瘤出现不同程度缩小,2/19/24例肿瘤消失。2、放疗可引起盆腔区域多个软组织,器官的异常核磁改变:(1)直肠、膀胱壁增厚;(2)脏器周围间隙可见条索状影,部分纤维化;(3)盆壁肌肉及皮下脂肪内可见异常信号。结论:MRI成像可显示放射线所致的盆部软组织器官的损伤,更重要的是有助于宫颈癌放疗疗效的评价。Objective: To study the radiation - induced changes of carcinoma lesion and non - tumor tissues and organs of pelvis in patients with cervical canner on magnetic resonance (MR) imaging during and after radiation therapy (RT). Methods: pelvic MR imaging was performed in 36 patients with cervical carcinoma before, during, and after radiation therapy. The size and signal changes of carcinoma lesions and pelvic soft tissues were observed on MR images. Results: 1. efficiency of RT: during/after/after 2 - 3months RT, tumor volume showed no changes in 2/1/0 patients, shrinkage in 32/16/12 patients, disappearance in 2/19/24. 2. Ab- normality was detected in multiple tissues and organs on MR imaging after RT. ( 1 ) viscera changes : the rectum and urine bladder wall was thick. (2) The clearance surroundings of visera has a stripe form and partly fibrosis. (3) abnormal sign was seen in the muscle of pelvis and in the hypodermic. Conclusions: MR imaging is helpful in the evaluation of cervical carcinoma response to radiation therapy and can detect radiation induced changes of soft tissue of pelvis during and after RT.
